Dd injured - not happy with A&E.....

Dd took a crashing fall from a height at sports training last night and has hurt her foot and unable to bear weight on it.

Went to A&E last night and they took X-rays and said her ankle was fine. They wouldn't listen to her when she repeatedly told them the pain was under her arch and across the top of her foot not in her ankle. Sent her home still unable to weight bear with no crutch's and a self care leaflet for ankle injuries.

Would ibu to take her to our local minor injuries unit today for a second opinion? She's on paracetamol and ibuprofen alternating every 2 hours and it's only just taking the edge of the pain. Foot is swollen along the outside and across the bridge and is very painful to the touch. She also has a high pain threshold so when she is crying in pain I sit up and take notice!

Dh reckons they will review her X-rays as a matter of course this morning and will ring if they've missed anything - is this right?

This is the correct treatment for what you have described, x ray will be reviewed and contacted with what they find. Pointless clogging up a miu they cant/wont tell you anymore than what you've been told. Follow the advice given and wait for them to contact you😊

I would speak to a pharmacist about swapping the paracetamol for cocodamol if she’s still in a lot of pain though OP. As well as using ice as suggested up thread.

You can take paracetamol (assuming you take the two tablet dose) four times within twenty four hours but you must have at least four hours in between doses. Realistically it will be more than four hours between some of the doses otherwise you will not stretch them out over 24 hours. So that is eight tablets overall.

You can take only six ibuprofen tablets overall in a 24 hour period, usually two tablets taken as three doses and you should have at least six hours in between those doses. Again, there will need to be more than six hours between some to stretch it out. You must take a decent amount of food before using ibuprofen.

You can use both drugs alongside each other, it doesn’t matter whether you alternate or if some doses occur with both types of drug at once. The key thing is that each individual drug is used correctly in accordance with the instructions.

If you think you have given your daughter too much or too frequent a dose please get medical advice.

Then with respect, it's a little concerning that you're giving out incorrect advice in regards to medication, to members of the public, on an online forum, if you are in fact a "paediatric healthcare professional". Whatever that may be specifically. I'm a "healthcare professional" also - I generally call myself a nurse, so am quite au fait with medications - and your advice is incorrect.

Just because you can have 3 doses of something within 24 hours, doesn't mean it needs to be spaced out exactly evenly ie 8 hours between each dose. You can do 6 hours between each dose and once you've had your 3 doses within the 24 hours then that's your lot for that period! Providing the space in between each dose doesn't exceed the recommended limit.

Pharmacists are a wealth of knowledge in all things medication related and in my experience are very happy to discuss any questions that staff have in relation to meds. It might be an idea for you to give pharmacy at your hospital/clinic etc a call to discuss this, as you could be denying your patients pain relief unnecessarily.

I've been told to alternate every two hours paracetemol and ibuprofen before by a pharmacist and a GP, also for DD1 when she was 16.

Sounds very similar injury to my DS last week. He could walk using his heel but was agony to put pressure on the ball of his foot. We had his foot x-rayed (and the doctor talked us through the actual x-ray which was fascinating) and clarified it would be sent for another option within a day or 2.
His diagnosis was a sprain and to ice it and elevate it and keep up with paracetamols. Estimated 10 days before doing sport etc.
A week on and its much better

Yes I would take her back. I broke my ankle years ago, went to A&E twice told it was fine and just a sprain and that I needed to "try harder"(I couldn't bear any weight on it) and couldn't walk. They wouldn't give me any crutches. Ended up going to the GP who referred me for an MRI scan which showed a fracture - took 3 weeks to get picked up. So I would be persistent, things get missed.

In my limited experience I have fractured (not broken) bones twice. Both times I went to A and E, both times I was x-rayed, both times I was sent home, both times I continued in agony, both times I went back and the second lot of x-rays confirmed the crack.

A fracture is a break. Lots of people seem to make this mistake. It is worth knowing that when a HCP says there is a fracture, or that a bone is fractured, it is actually broken.

your DH is right in thinking the X-ray will be reviewed by it might be a couple of days.
I would be either taking her back or to the minor injuries unit you mentioned.
This happened with my niece - sent home and then rang a few days later as it was actually broken and in the same area you describe on your daughters foot x

Some years ago but my DD had an accident, Dr in A&E said no breaks and she was just making a fuss. I was told to encourage her to walk and sent home.

A week later got a phone call asking me to take her back for a review the next day (Friday) where we were greeting with a wheelchair and taken through to a private room and shown her xray and the fracture pointed out, it had taken a week to review it. Then told to come back on Monday when a Consultant would see her. I said we weren't leaving. I was told we had to, no Consultant available. I said as she had been walking in pain for a week on their advice I was not leaving until a Consultant saw us and reviewed everything. Lots of tutting then doctor left and came back to inform me we were very lucky as one of the Consultants in fracture clinic had agreed to see her in his lunch hour.

Great, so off we go to fracture clinic where Consultant shows us the same xray and points out a different fracture. Yes there were two.

I made a formal complain but as a nurse lied to back up the original doctor we got nowhere with it.

Don't hesitate is my advice, get an xray today. I hope you get some help.

I would recommend going to minor injuries as I found them excellent for this kind of thing. My friend recently went to A&E after an accident and they checked there was nothing serious and sent them home. They went to minor injuries 2 days later and turned out that they had snapped a small ligament which needed holding in place to mend for 6-weeks.

Obviously it's better that they went 2 days later than not at all but it hasn't fixed in place properly and I suspect this was because of the delay. Minor injuries department were brilliant and even said they could pop back in after a couple of weeks to check everything was ok which they did. It isn't just broken bones which are a problem - soft tissue injuries can be worse.

Jesus. No. Ibuprofen should be every six hours and not more than three times in a 24 hour period.

Absolutely take her back, I hope she's been seen by now.

I've broken the bridge of my foot, twice. The first time was picked up the day I did it, the second time I had to sit and basically throw a tantrum to get it x-rayed. The triage nurse said it's impossible to fracture a foot in the manner i had previously and went to send me home. I talked them into sending me for an x-ray and lo and behold I'd done the same injury again.

If your dd has turned her ankle she could have sustained the same type of fracture in which case x-raying the ankle won't show it, she needs the actual foot x-raying.

I agree with take her back.

It took me 3 times at A&E with dd when she was two for them to discover the broken leg.

It was only a minor fracture and didn’t need a cast but it was missed twice and explained why dd was in so much pain and not weight bearing

I think you're getting muddled re painkillers. You don't need to wait 4 hours after giving paracetamol to give ibuprofen, you could give it 2 hours after giving paracetamol for example. Our GP has always said you can alternate but you must always be mindful of limits. Work out a schedule that might help, or pop into the chemist to see what they suggest.

Re the xray, things get missed all the time so I'd definitely go back.

I was told to take ibuprofen not paracetamol when I broke my foot. I was told paracetamol isn't recommended for fractures but I can't remember why.

Sorry should have said dd is 16.

She says when she was xrayed the red lines were on her ankle so from that would assume the foot hasn't been.

She had 2 doses of paracetamol and 2 doses of ibuprofen since 10pm last night so still within the daily recommended doses for both.

Thjnk I will take her in again this morning - I strapped her foot last night and dug out an old pair of crutches for her to use - we were sent home with no strapping and they watched her hop out of A&E Sad

The red lines were on her foot. Even if an ankle problem is suspected, they will x- ray the whole foot.

Sometimes x-rays can't give a clear picture when there is swelling and a hair line fracture. If your daughter can't still put her foot down because of the pain, she needs an MRI.
