to not trust this GP surgery?

Filling out the new patient registration form and It doesn't ask if I've ever been for a smear or when, or what my history of pregnancy, miscarriage, or terminations are?

Actually, it doesn't ask for my sex anywhere on the form. Angry

And no, it doesn't ask gender or gender identity either.

Anyone else find this on a new patient registration form? AIBU to think this is a bit shit?

No...? You put information about your last surgery on the form and they request and receive all of your information from them?

Maybe the form is literally just to register at the practice and they’ll take history afterwards?

Should they be asking those questions? When I moved surgeries they didn't ask that either. Maybe they asked which sex I was but not the others.

I’ve never known a GP’s registration form to ask those questions.
I've never known one not to! I even checked my current one's page to confirm I wasn't going mad.

I vaguely remember filling out a new patient form for DP, I think it asked name, address, possibly date of birth, and previous Dr's surgery - presumably all medical and personal info is on the records they request from your previous doctor?!

Its a standard form ? Your notes will be transferred. Lots of people would be really upset being asked that on a registration firm.. it just needs your admin details and some basic health/lifestyle questions.

They ask about all the other stuff though. Skin conditions/ asthma heart conditions. It's just like every other one I've ever filled out minus the stuff about female history.

Were you asked for any details like your NHS number, full name and date and place of birth?

Also many GP surgeries require an initial appointment to collect more data.

You realise that your whole medical history will be requested by your new surgery once you’re registered? The forks are more for the ‘right now’ details and facts that may not already be covered in your medical records (eg family history of health etc)

Unless all my previous surgeries have been weird, I've moved around a bit and been asked on all of the ones before. Have a look at the new patient reg on your local surgery I bet it asks those questions.

Agree with PP- they just need enough to be able to get your records from your last GP surgery.
If you have never had a GP surgery before due to eg recent immigration, then they will likely contact you once they find out they can't get your records to get a fuller picture.

They're only asking for information they might need before your medical records are transfered over. They're not starting your medical records from scratch. YABU.

Hard to say really.

It doesn't but you have to fill out the GSm1 (I think that's the form) which is the standard NHS form that everyone uses for transferring your info.

But your normal practice registration form has all your history and family history. Oh and units of alcohol. Which this does. Except for the women-specific stuff.

Your previous surgery/ies may still use a lot of paper/paper notes. It varies between surgeries but I certainly wouldn't take it as a bad thing that you didn't get presented with loads of paperwork.

If you think you are due a smear call them up and ask

Yes, but they still always ask for history, meds etc themselves. It would be silly not to incase anything gets missed

Also smear and breast screening recalls are generated centrally so the practice doesn’t need to set them up. Whereas yearly recalls for chronic disease screening eg diabetes are done by the practice so they would set up recalls for new patients.

I filled one of these forms in recently. It's a standard form. It asks for name, address, NHS, previous surgery details etc but nothing about your health history. They get that from the previous GP.

They're still asking that in other surgeries and they still asked for loads of private info. Just not sex. Also, google is telling me that they're actually supposed to start asking about sexuality too so m/f certainly isn't too personal.
