to not trust this GP surgery?

Filling out the new patient registration form and It doesn't ask if I've ever been for a smear or when, or what my history of pregnancy, miscarriage, or terminations are?

Actually, it doesn't ask for my sex anywhere on the form. Angry

And no, it doesn't ask gender or gender identity either.

Anyone else find this on a new patient registration form? AIBU to think this is a bit shit?

Honestly I don't think you need to read too much into the form. The 2 things that will survive a nuclear hollocaust are cockroaches and NHS beurocracy.

I'd reserve judgement for whether the doctors/other staff seem nice, competent, etc.

Yes had the alcohol units of shame on there! I was able to say 0 because I was pregnant...

Perhaps the areas you lived in had a lot of people who were new users of the NHS so the surgeries requested medical history from all new clients as it can be very difficult to obtain medical records from abroad.

I just looked on my current GP's website as I'd forgotten what I was asked.

It asks for the date of last cervical smear, but doesn't ask about pregnancy, births, contraception.

It asks if I suffer from 10 chronic conditions including depression and COPD but doesn't ask what medications I'm on...

Sounds like there is a very large range in the amount of info they require. Which is surprising - I'd have thought it would be standard across all NHS surgeries.

I wouldn't say so, but I am prepared to believe there's obviously a lot of disparity in the forms from the responses to this thread!

I’ve just looked at our practice one as you suggested. It does ask about my last smear but nothing about pregnancy - I’ve never known a surgery ask all that on the forms as it’s not hugely relevant at that time.

Yabu, I’ve never filled out a registration form that detailed they just ask name/dob/address etc. They just need to register you. Why would they ask you about miscarriages/terminations on a general registration anyway, I think the midwife asked those sort of questions when I was pregnant but my GP didn’t Confused

I’ve never been asked to fill out a form detailing any terminations, miscarriages or how many smear tests I’ve had etc this will all be on my medical records.

The pt qu is important for current issues that may require current medication.
Your smear etc are unlikely to be due the day you register and this detail will transfer from your past surgery.

But, if you don't trust them- register elsewhere.

The registration form is just taking your name address NHS number and contact details. When they accept you as a new patient they will contact your previous GP and have all your previous medical records transferred to the new GP practice . So they'll know all about you.

But they just request your medical records from your old surgery and that will have all your info / medical history / info on it surely?

It asks for asthma and medication so they know if you need medication in the first few weeks what's what. At that point they would also request info quicker from previous surgery.

Full notes transfer electronically so they will be able to see your sex and smear status ect straight away.

That's fine. But it also asks about

Diabetes
Skin Disease
High Blood
Pressure
Nervous Disorders
Heart Attack Congenital Disorders
Stroke Cancer
Epilepsy/Fits Allergies to
Medicine/Food
Asthma Any other Disease

How many units of alcohol I drink

and if I am an ex smoker the details of what I smoked prior to giving it up Which I am fairly sure is non-emergency.

And as I said, I have been asked the other questions every time before so even if people think I am being strange. My previous GPs did also agree these things were important.

In fact, while I disagree with using the term "gender" in medical forms instead of "sex", I'd be happy enough with that as it would at least get an accurate response for the sex for most people. And women would get called in for things like smears.

Oop sorry I should say it asks if any family members have those conditions. Which means not an emergency this week. It's about finding out all my medical history.

Or do you not think it's more relevant to my medical history to know if I've had multiple miscarriages and 6 live births than to know if my Aunt Ida has ezcema?
