Hospital wants to send me home after PPROM. I don’t want to go. Help.

Posting for traffic. My waters broke at home and were bright red, and I was 26 weeks with DCDA twins. I called an ambulance and was rushed to hospital where I had a huge gush of red fluid. I was counselled about preterm birth and given steroid injections. I saw a neonatal doctor who stated the most important thing was to deliver at a hospital with NICU cots.

Despite severe agonising pain, a week has passed and I haven’t given birth. Twins aren’t distressed and my observations are fine.

However I feel incredibly unwell. I keep burning hot and turning red but my temp is not raised. I feel like I have the flu or another illness. I have no energy and can barely move due to pain. Codeine won’t touch it. A midwife told me to have a hot bath which I am sure is contraindicated.

The staff want to send me home. I have stated I do not want to go home due to me living an hour from the hospital and the risk of infection, cord compression, preterm delivery and stillbirth. I do not want to lose my babies. I want to remain in the unit with NICU like the doctor advised.

Can anyone give me any advice? I can’t find any info on this anywhere and feel like the staff are fobbing me off.

If the registrar is not answering, they have an obligation to call in the consultant.

I'm bleeding quite heavily. It's literally running like a tap and I can't move anywhere without it getting worse. They've just...gone. Me and my partner are here unsure what to do now.

Yes we are still keeping records. At night/weekends are always when it kicks off.

Is the vaginal loss fresh red, in the same way as if you cut your hand and bled, or mix in with discharge and/or mucous (jelly)? Have you got a pad on?

Bright red like period blood. Got a pad on but it's just soaking it in minutes. Graphic photo but this is what I woke up to. This is what is not being taken seriously...

That's on my bed square after going through my pad and my underwear in about 10 minutes. No one seems to think its worrying except me.

I wish I could help more than writing a message on an on line forum. That needs reviewing by a doctor ASAP. Even if that has waters mixed in with it that would concern me very much so as a professional.
It may be that if the fetal monitoring is okay, that the doctors are happy to continue to observe at present, but either way it is not midwife’s place to say that that is normal. That is outside our scope of normality.

I cannot comment on what plan will be put in place for you, as that is unprofessional of me to do so without also having the rest of your medical history, but I can definitely say that I would want that to be reviewed by the doctor.

I’ve only read your last couple of messages but want to suggest you put the emergency buzzer now. Not the normal call bell. You should absolutely not be left when you are having heavy fresh red bleeding. Are you on labour ward? It sounds like possibly not?

Soaking massive period pads every 5 minutes. Luckily a midwife who knows my case has put me on CTG and taken the pads etc to weigh and is bleeping doctor again.

I'm on the antenatal ward in my own room. They are listening now luckily. Babies don't seem distressed.

Midwife and NICU nurse here... it looks like an APH (antepartum haemhorrage) perhaps from the twin whos waters have gone. You should be on labour ward with looking towards imminent delivery!! when were the twins last monitored?

On the CTG now. I have been bleeding like this on and off for days now and keep getting told its fine but I feel like absolute shite. My skin is white and grey and I have no energy both of which I'm told is because I'm losing blood.

Very concerning. Id be incredibly surprised if they dont deliver the twins asap!
How are your own observations in terms of pulse rate and blood pressure?

Bp is 106/65 which is normal for me but unsure about pulse. Bleeding won't stop

They must act on this. You at least need closer observation on labour ward, even if they don't want to deliver the babies.

I don't deal with babies until after they're born(!), but that does seem quite a bit. I'm not saying whether or not they need delivering, but I'm slightly surprised you're not being observed on labour ward, where there'd be a higher ratio of midwives, and where the doctors tend to get stuck out of hours. Does sound like you need to have a medical review.

The PPROM (preterm prolonged rupture of membranes) isn't the biggest worry. You have to guard against infection, and if liquor levels drop too much it isn't great for the lung development, but often they're fine for quite a number of weeks. It's the ongoing blood loss that has people a bit twitchy.

Doctor has seen it. Speculum showed cervix closed and bleeding isn't active? So doc said it should stop in 5 minutes but 15 mins later im still soaking these massive makeshift pads they've made me so they're getting the Drs back.

Had severe pains today and getting them now. No dilation though.

Your heart rate is a bit high. It could just be up because of worry, or pain. Or it could be a sign of the blood loss. They do need to be checking it regularly.
