If a GP decides to redact information from a patient as it 'can do harm' to the patient, what probable scenarios is the GP alluding to? Also, they are trying to block the patient from accessing the information.
AIBU to be confused why the GP doesn't just disclose?

You sound way over involved in this. Does the lawyer specialise in medical issues, if you are so sure the lawyer can obtain full unredacted notes then why haven't you just instructed him to do this. What exactly do you want us to do. This seems more about criticising the GP than helping your relative. Maybe it would be fairer if an independent advocate got involved,

Am I being pedantic (and hope not goady!) I may have missed an answer if already asked! How does the patient know there is information there, if they don't know what it is, or who gave it?

You can get wrong information corrected, you can put a note on to say you disagree but only in exceedingly exceptional circumstances can you get info permanently deleted from the record.
