10mth old screaming & straining ALL day....what to do???

Hi

My 10mth old DD has been diagnosed with a milk intolerance and she has been prescribed wysoy which she has been on for a couple of weeks.

At first she seemed ok, then a few days ago I gave her a soya yogurt which set her off screaming/straining etc and ever since then seems unable to tolerate her soya milk.

We referred to the hospital a few mths ago prescribed soya milk in the first place and the earliest I can get another appt is Jan

I have a GP appt on Wed for her, again the earliest I could get

What do I do in the meantime? DD was so bad this evening I had to help her poo with tissues etc (sorry TMI)Once she has pooed she is quite happy.

Do I continue with soya milk (screaming will do my head in and she must be in pain) till I see someone

Or do I put her back on "normal" FF which she was intolerant to but nowhere near as bad as this

Or has anyone got any other ideas?

Thanks so much, just hate seeing DD in pain

right so to prepare you need your facts and data to bring with you. It is hard to tell a mum she doesn't know what she's talking about when you bring in a detailed history of your dd's health problems.

So....

Write down all problems she has and when they started (e.g. what age). Write down the foods you suspect are a problem and the associated symptoms. e.g. drinks dairy formula, vomits, diahhrea, screams all night, etc.

Have any reports that have been done already with you, plus any observations by other medical professionals.

Have a very clear idea as to what you want to accomplish at this appointment, e.g. I want referral to an allergist and immediate script for neocate or other similar formula. Listen to what the dr. is saying to you but if you get the impression that he\she is patronizing\trying to rush you out the door\an idiot, stand firm. You need to advocate for your daughter. First approach is softly, softly, give history, tell symptoms and then ask what he\she suggests. But again if you're getting nowhere, you might have to be a bit more firm.

hope it goes better this time for you and I'm totally with noiamnot on the be very prepared idea. it's easy for paeds etc to try and dismiss what you are saying so the more info you arm yourself with the better.

much more difficult to fob you off then iyswim?

Haven't read the replies but I do know that soya intolerance goes hand in had with dairy intolerance. My neice developed lactose intolerance after Gastro-enteritis and got better for a short time on the soya milk and then got worse again and developed hives. In they end they went private because they didn't get anywhere with the GP. They ended up with a prescription for Neocate.

Ah, just read the other replies - my niece hated it too - they flavoured it with a tiny amount of nesquik then gradually reduced the amount to nothing.

Noiamnot has very wise words....go prepared. You have to take the lead in the appointment with the paediatrician. Remember that you know your daughter the best, so if you feel you are being fobbed off, stand your ground.

There are a number of strategies which the paed can suggest for cow's milk allergy. The 'gold standard' treatment is exclusion of cow's milk in your dd's diet ie. addressing the root cause of your dd's problems. For a ten month old this requires the prescription of a cow's milk free formaula ie. Neocate (or a very similar new formula called Nutramigen AA), and dairy free foods in her diet.
At the other end of the scale, minimal intervention would be the treatment of the symptoms ie. lactulose or senna for constipation, and creams for your dd's sore bottom.
The intervention your paed decides is best will vary on their experience and confidence in treating allergies. IMO they shouldn't p** around, and go straight for the best treatment.

Noiamnot - will definately do list. Do you think I should be pushing to see allgerist? Don't think she has a problem with anything else TBH but maybe they are the ones to talk to re trying to re-introducing milk somewhere along the line.

Jacanne - My DD was the same on soya in that for the first week she was much better and then things changed overnight for the worse..lots of screaming/straining etc.

Klick - I definately got fobbed off last time...I won't be so naive this time

DD has also been slow to do things ie she only sat up a couple of weeks ago at 9.5 mths, is not crawling yet (although I know some babies don't) and GP said its probably because she hasn't felt well so didn't want to do much iyswim which makes me very

push for allergist. You are already seeing a problem with milk and soya. You should get some definitive answers via allergy testing.

Sarsums - some stuff arrived today I didn't realise she couldn't have bread either

Is there any bread out there that she can have? I also give her bread sticks as snack...

noiamnot - thabks for advice. Letter came from hosp today ans it doesn't say have a appt with a specific person just says joint neonatal team.

simpsons it will be a paed on the nn team, but please don't stand for excuses, being fobbed off or anything like that.

you do have to push and push and push with them to get pretty much anywhere but it is worth sounding like a snotty cow for it i should know!

You only need to avoid milk and soya if there is a milk or soya ingredient in the bread. There should be a list of these ingredients in the booklet that I sent you. Just check the ingredients list on whichever bread product you are using. Sorry, I don't have the booklet infront of me right now, but I will look at it on Tuesday when I'm next in work.
At your next appointment with the paed, I think you need to ask to see a dietitian. I'm sure you will have many questions like this regarding the everyday practicalities of the milk/soya free diet, which a doctor may not be able to answer. I will happily help you out with any questions, but it would be better to see someone face to face.

Well...the neocate has arrived so am going to brave it and try DD on some

Think I might try half and half first. She only has 4 scoops per feed so 2 neocate and 2 cow & gate and see how we get on

She drank half of the bottle which is not that unusual ATM as she is a bit sniffly.

Her breakfast & bedtime bottle is the main ones TBH.

Do you think I should do 3 scoops neocate and 1 scoop cow & gate for that one?

i would go with the revised amounts - see how she goes. worst case you just have to make up another bottle i guess.

Flick - she went to bed at 6pm and I gave her 3 scoops of neocate and 1 scoop of C & G and she drank 3ozs out of a possible 5 so not too bad

The big test will be tomorrow morning as she will not be so sleepy!!

Good to hear that she's taking the Neocate mixed with C&G. Hopefully she'll take 100% Neocate. Just a warning that her poos might turn a funny green colour. This is nothing to worry about, it is perfectly normal .

Well hospital was hard work!!

DD was seen by 3 peads (each time stripping her naked etc) and then they called the top consultant (who turned out to be the knob I saw last time)

They are convinced that she is suffering from constipation

At one point I lost my temper and said "you are not listening to me!"

However good points from today are that she has been referred to a dietician and that I have a prescription for neocate although I would not have got that if she hadn't been on it already!! So cheers Sarsums!!

of course there is the possibility that there is constipation with a big blockage and all the runny stuff is "leaking" round it iyswim, but the issue is what is causing her constipation - that's what they need to sort out!

That's great news, shame about the paeds. And good to hear you've got the dietetic referral! How is your dd getting on with the Neocate? Have you managed to give her 100% yet?

Thanks guys!! Feeling totally drained am hitting the wine tonight!!

Am putting DD on 100% neocate tomorrow. I have to ring the consultant on Friday.

The consultants plan was to put her back on soy milk but give her something like lactulose. Grrr...

She has anal fissures which are causing her pain too

A quick question...surely if it is constipation then if a food is causing it then its an allergy?

Or could something else be causing it??

i guess that would be the case - my point pretty much - even if it is constipation they need to sort out what is causing the constipation.

good luck with the 100% neocate. went into the pharmacy for something today and they had just had a delivery - must admit that i did think of you and your dd
