Any doctors around? How fucked am I?

I had an MRI on my liver due to some lesions found on a totally unrelated ultrasound. They suspected NAFLD given my weight etc but instead They’ve found something more worrying, but the GP I spoke to couldn’t give me a straight answer as to what the MRI report means. Is this most likely benign or most likely not? GP treated me like I was going to crumble but I’m a very pragmatic person, I just need to understand what I’m looking at here.

Im 33, I have a 2 year old DS and I just need to know how worried I need to be here instead of waiting another few weeks for the next MRI and results from that. If I’ve got liver cancer I know how bad that is and I need to start getting ducks lined up and quacking sooner rather than later.

So if you’re a doctor/ medical and you read the below report, what would you be thinking? You can be honest, I’m completely aware no one can diagnose me from the below, I just need probabilities and ballparks.

Any doctors around? How fucked am I?

I’ve just seen that you said that your bloods were fine also. This is also a very positive sign. All my relatives and friends who had something more serious, such as cancer, all had some kind of markers in their blood as well such as raised white blood cells.

I just wanted you to feel more positive in the wait. And as others have said, most incidental referrals if ‘something is seen’ are usually benign. So if you have normal bloods, no symptoms etc then this is all pointing to the positive.

I have had a couple of ‘things’ picked up from symptoms / MRIs and the wait for ‘confirmation’ was awful. One of them they still can’t tell me what it is, kidney related, and I am checked every year to see if it has grown and I’m kind of used to it now. It thankfully, hasn’t’ grown!

My mother's x-ray for chest trouble and coughing stated "possible carcinoma".

I'm not making this up. 🙄

She was then referred for a PET scan which confirmed it.

Yes you're totally right. Sometimes, cancer just looks like cancer and I've reported many chest x rays ending with the sentence "appearances are highly concerning for lung cancer."

Sometimes, it isn't as clear in which case the wording can be less committal.

Morning all, my contrast MRI results appeared in the app:

Clinical History :
MRI liver with contrast:
Previous MRI from 12/01/2024 has been reviewed.
Previously seen four liver lesions are again noted:
-Segment 8 lesion peripherally measuring 26 mm, segment 6
lesion measuring up to 35 mm, segment 7/6 lesion posteriorly
measuring 31 mm and a segment 8 lesion centrally adjacent to
the IVC measuring 17 mm.
These are hypointense on T1, mildly hyperintense on T2, with
mild diffusion restriction and no significant intralesional
fat component. Rather homogenous postcontrast enhancement is
seen in the arterial phase, lesions appearing more or less
iso-to mildly hypointense on the venous phase with central
scar enhancement on the delayed phase. Findings are
suggestive of or FNH.
No new liver lesion seen. Hepatic veins and portal venous
system are patent. Hepatic margins are smooth.
Spleen, adrenals, pancreas and kidneys are unremarkable. No
para-aortic lymphadenopathy or ascites seen.
Previous cholecystectomy noted. No intrahepatic biliary
dilatation seen however, there is mild prominence of the CBD
which measures up to 12 mm, with a filling defect in the
distal CBD which measures up to 13 x 7 mm, with mild smooth
wall enhancement of the distal CBD likely reactive.
Impression:
Documented for liver lesions have enhancement
characteristics suggestive of FNH.
Distal CBD filling defect likely represents
choledocholithiasis, with reactive smooth enhancement of the
distal CBD. Correlation with liver function tests and
consideration for ERCP recommended.

Any doctors please correct me if I’m wrong but that reads to me like:

  • I have FNH which means the tumours on my liver are benign
  • The filling defect in my bile duct is a gallstone stuck in there and they need to do an endoscopy procedure to possibly stent the duct to stop it causing issues?

Are you on the pill by any chance? Apparently there’s a link and my tumours shrunk after I came off the pill when I started TTC.

Lesions and cysts on the liver are SO normal and pretty much always found by accident (often during investigations for gallbladder issues). I had a HUGE one which i had surgery to remove last year.

Its so scary though! Glad everything seems to be ok :)

It doesn't state "possible carcinoma" so I don't think it's suspected cancer.

It does list liver mets as a differential though, so your comment isn't accurate.

No I’m not on the pill but I’m all kinds of hormonally buggered so it could well be that!

This is what I had! An FNH. It happened within a year of me being pregnant, and there are suspicions it could be linked to hormones like the other poster said. However, because it is benign, no-one is ever going to spend money on research to find out.

I discovered mine because I was having mild twinges of discomfort near my stomach, and could feel a lump about the size of a golf ball. It has completely disappeared now.

I hope you get full confirmation very soon. I remember how bloody terrified I was at the time and I’ve been thinking about you. Lots of love.

That's interesting, DS is almost 3....

On the bile duct as well i bloody told them 2 years ago i had gallstones again even though i dont have a gallbladder and was told it was trapped wind because 'forming stones in your bile duct is exceptionally rare' - well bollocks to that!

So glad I found your post OP. I have had a blood test with one liver result very slightly out of kilter. I have pain in the bile duct on eating. I had a bile leak and stent after having my gallbladder out and have had pain in the past. Now it's much worse. I know I get sludge as my stent totally blocked with it.

Second blood test today and ultrasound at the end of the month. I'm so scared because a friend died in weeks of bile duct cancer. I feel sick a lot and can't eat much, but no jaundice or other symptoms.

I'm hoping that it's sludge or a stone. I'm feeling a bit reassured having read your post.

That’s exactly how I feel when my bile duct presumably blocks - I get biliary cholic and feels utterly awful until it passes.

I was explicitly told after I still got the pain after my gallbladder was removed thst it couldn’t possibly be stones in the bile duct but I’ve just read that 1 in 7 people who have the gallbladder removed will still get stones or sludge in the duct!

@Metabolicallycomplicated great news! You've joined the FNH club! So glad the wait is over for you and all worked out well. Yes they'll probably do endoscopy and fish out the stone, unlikely you'll need a stent.
