Wisdom tooth removal no sedation

I have a wisdom tooth removal booked tomorrow. I have two bottom impacted wisdom teeth, one on each side, one extremely near the nerve. I am a very nervous dental patient but nowhere, including the hospital, is doing sedation due to covid. I have 4mg of diazepam to take beforehand. Any positive stories to help me go though with it? I am on my 5th round of antibiotics now so I shouldn't really delay, especially as there seems to be no date for sedation coming back. All positive experiences appreciated!!!

You may be better off without the sedation to be honest. I was given midazolam before mine were removed and once the local wore off I was in agony - think rocking in the car in tears begging dh to drive to the nearest pharmacy for the strongest painkillers available agony, then after a few hours the pain suddenly eased - I later found out that midazolam actually increases your response to pain and when I felt better must have been when it wore off. I’m not sure what sedation they use these days (was 12 years ago next month) but I would do it sedative free if my top ones ever need doing!

Midazolam is also meant to make you forget the procedure - I can remember the whole thing so that’s not the case either! Horrible stuff!

Are they completely impacted, as in buried deep?

My bottom two were deep and horizontal. had local anaesthetic and both out: the procedure was ok, but had a lot of pain in my jaw and face joints afterwards. Bad headaches. Had a week off work. For me it was worse than C section recovery!

Hi OP, I spent a lot of my time as an Oral Maxillofacial nurse. No dentist will consider oral surgery under local unless it is a mesio-impaction which is an easier extraction and every patient will be told that the inferior dental nerver runs closely along the jaw where your tooth is.

No advice as I've never had it done but I'm also a terribly anxious dental patient and can completely relate to your nerves. I'm sure you'll be fine, the thought of these things is always so much worse than the experience itself, it's the fear of the unknown. Focus on your breathing and maybe wear some earphones to distract you. Good luck. X

I’ve had two wisdom teeth removed with no sedation, just injections. I agree it’s like the dentist is pushing down. I’m very nervous and the tears stream down my face. I count the prime numbers down from 100 to disassociate.
One came out fine, the other (years later) in three pieces and that was tough going. Nobody explained about dry socket, which I got by worrying the wound with my tongue. It was extremely painful and the smell was horrendous.

I'm a dental nurse in the dental hospital so we see the complex cases. We have so so many nervous patients. They all cry at the end because they're so relived and thought it was going to be 100x worse. If it's happening in a dental surgery they would only do it if they were confident itl come out with no issues, if it's in the hospital it's all they do day in day out so either way you're in good hands!

I had a wisdom tooth out and it was fine. Bit sore where they had to pull and move it around that's all. Had a molar out later and that was worse ended up infected, get painkillers and if it feel like it's getting worse after, get checked straightway. and remember a small bit of pain now will make all the difference next week when you don't have any pain

Just be prepared for cracking noises and wiggling sensations. At no point after the local does it hurt.

I’m getting more nervous of the dentist as I get older. It’s nearly 6 years since I last went 😳. Also as I get older it seems I can’t open my mouth as wide. I know I need work doing on a back tooth but can’t imagine them being able to get to it with any tools. Also my gag reflex is huge. Just writing it down I’ve given myself palpitations.

Any advice?

My GP recommended I didn't take sedation, and this was long before,COVID-19. She said it was better to remain conscious, but have the local anaesthetic.

I'm glad I took her advice, everything went well, it was out before I knew it.

The most important thing is after care, take all the advice they give and follow it to the letter. As already stated upthread, dont swoosh the warm saline water, very gently hold it in your mouth against the site of the WT and spit out carefully. Don't do any exercise for at least 3-5 days, to ensure the clot doesn't dislodge.

I’ve had all 4 wisdom teeth removed but it was under GA as they did all 4 at once and they were impacted. So I can’t comment about the pain itself but for post operative pain, can’t the dentist write you a prescription for a short course of dihydrocodiene or a stronger dose of cocodamol? Worth asking - even if it’s just to get you through the first 48 hours after the numbing injection wears off.

If you can’t get them to do that, Solphadeine Max (over the counter) is 12.5/500 so slightly stronger.

Let us know how you get on, and don’t worry if you look a little like a hamster for a couple of days! It is totally worth it in the long run.

Dd (23) had impacted wisdom tooth removed a fortnight ago. She’s usually sedated but apart from looking like Mike Tyson has punched her in the face (due to her EDS) she’s fine and was just relieved it was out.

I had local anaesthetic only when I had mine out (all 4 at once). It really wasn’t too bad, but the next day was pretty painful!

Hi OP,

I just wanted to wish you luck, I hope it all goes well for you.

I had my wisdom teeth removed under G.A, I know this is not what you are having. I just wanted to say if you are prescribed codeine please be aware that it can make you extremely constipated. I had this after my teeth were removed and the pain I experienced with 3 days of severe constipation far exceeded any discomfort I had with the teeth removal. Sorry if TMI but I just wanted to say this in case you aren’t aware of the possible side effects of codeine. If you take it I would suggest taking something like dulcoease or equivalent alongside.
Also ask about antibiotics to try and avoid any kind of infection afterwards. I know you said you have been on several courses of A.B’s but you ideally need some for after the procedure too.
I found my mouth healed up very quickly afterwards and touch wood I’ve had no further problems since.

Thanks folks! I have had one wisdom tooth out - hurrah! The other one is horizontal so he reckoned I'll need a general anaesthetic for that but he thought the pain on that side might be caused by another tooth rather than the wisdom tooth so I need to go back to the dentist when this one's settled down. It wasn't as bad as it could have been. I asked for the radio on and did maths sums in my head. The diazepam wasn't that useful though - I wonder if the dose was too small as I was still shaking like a leaf!! Managed to somehow not hear any cracking sounds. But yay, very happy to have had it done and will certainly bear in mind the codeine and constipation issue!! Really appreciate all the support in this thread Smile
