How long do I leave DD in pain?

Sorry this could be a long one.

DD (9) has had pain in her upper right quadrant for nearly 6 weeks now. I took her to GP originally after it was getting worse not better - was sent to children’s ward who did bloods and sent her on her way with the surgeon saying she’s constipated, bloods were fine.

The next night she laid on the floor screaming for 20 minutes unable to get up so she went up again. This surgeon said it was muscular & to rest for 2 weeks.

Pain was still as bad (think screaming out with every step) nearly 2 weeks later so took her back to GP - he gave her exam saying yes definitely muscular and just a case of painkillers, which don’t touch the pain, and rest.

4.5 weeks after the pain started and she was still in as much, if not more pain. GP sent us back to children’s ward. More bloods and an ultrasound didn’t find anything. Dr said she could feel a couple of hard stools at the opposite side of stomach so as they’d ruled everything else out it was most likely that. Sent home with movicol.

That was a week ago and she’s worse now than ever. It used to just be when she moved it was sore, now it’s when she’s lying still, much worse when moving though.

I feel like we’ve seen enough dr’s that I should be reassured- but I’m not. I worry if I take her up again I’ll be labelled as a time waster/exaggerating. So wwyd? She’s been referred to consultant for appointment in 6 weeks - do I let her scream for 6 weeks? 1 more week? She’s opening her bowels daily and they’re soft.

Go back to gp.
If you don't the gp will assume she is better.
Are the bowel movents normal/ soft or runny?

Upper right quadrant is gall bladder territory. How old is she?

I would go back and tell them she's not constipated and if they can feel something there it needs investigating

She's 9, in all seriousness it could be the ovaries starting to kick into action and she has a cyst.

Many medics ignore that ovarian cysts are possible in young girls, cysts are very common they just rarely cause problems. The very start of puberty is actually a time they are likely to occur because the hormones are all over the place. Also hormones can cause constipation.

Another thing to be aware of is diverticulitis- stools/food get trapped in diverticula rand gets infected.

She’s 9. She pointed to a constipated stool on the chart to dr, she went every 2-3 days and took lactolose. When we came out she said none of the poo’s looked like hers so she just pointed to one - I’ve spoken to her to make sure she doesn’t do that again.

Now she’s on laxido she’s going daily and they are very soft - I’m obviously not involved in cleaning her up etc but the bowl is very messy afterwards. She got worse since going on the Laxido actually.

She is going through puberty - but would a cyst etc not have shown up on scan? The only comments made were scarring on kidneys which is unrelated and harmless, and a large bowel.

They ruled out infection as no other signs of illness and nothing showing in bloods.

Not always, especially if they weren't looking for one.

I would be going back as a matter of urgency tbh. It could be extreme bowel spasms (horrifically painful IME) but the pain sounds severe and getting worse?

Liquid stools can bypass hard stools that have got stuck.
A plain Xray is better than a scan to diagnose impaction. Have a look at the ERIC website.
That said, URQ is an odd place for impacted stool to sit, unless it extends all the way across and down as well.

It’s frustrating as every time we’ve gone to hospital she’s been largely fine, so they aren’t seeing her at her worst.

It’s worse today for sure, she also seems to have a bit of a fever BUT so do her siblings as they have the cold. I’ve just gone over her discharge notes again and it says to phone ward with concerns if Laxido not working. So I guess that’s my first port of call.

Every time I’ve gone up I’ve been made to feel like I shouldn’t have gone up, apart from one lovely dr.

I'd get her checked again. I went through something similar when I was 13 ish. They found d a giant ovarian cyst, removed it but I was still in pain. Then they decided to remove my appendix despite thinking it wasn't that and found my apprndox had grown huge too and was wrapped around my other organs. But for 2 years I was told there was nothing wrong despite being g in agony. I'm so glad my mum believed me and kept pushing.

Hope she's OK and it's something easily fixed

Perhaps phone and ask to go now as she is having an "episode" so they can better detect what us going on. Cysts can come and go or you know shrink and grow.

I just know by the time we get there she’ll be not too bad though - gah! Thanks for replies everyone I think I’ll wait til morning (she’s generally sorer then) and explain, rather than go up at night. Unless she gets extremely worse.

Appendix was mentioned but they couldn’t see it in scan and didn’t show on bloods, I know that doesn’t mean it’s not that though.

Hi, thanks for asking. She’s he same. Phoned the ward and got told off for phoning them - apparently dr should not have told us to do that. Spoke to a nurse briefly and she said she’s be sorer not BECAUSE of the movicol. I feel I can’t win. It’s exactly the same pain before in the same place, so shouldn’t be cramps.

I’m awaiting a call back from the GP. So they say her pain is from constipation = prescribe meds to help. Gets worse = blame meds. I bet once I stop the Laxatives they’ll say still constipation Confused

Sorry was too angry to proof read! Nurse said she’ll be sorer NOW because of movicol.

I’m so upset on behalf of DD. GP may well say wait couple days off meds and see what it’s like. But I feel like whatever we do it won’t be taken seriously.

Liver/gallbladder issue caused this pain for me. Might be worth being very pushy about liver function test and blocked bile duct test.

We’re you in an out zoo or did they pinpoint it straight away? I feel like they think we are exaggerating. I’m not a confrontational or pushy person but think I’m going to have to be.

A few weeks of being pushy. Suspected crohns at first. Actually turned out to be something very rare so unlikely what your dd has, but in that area of the body I would be checking to see if there are liver bile duct issues.

Just so pleased they are taking it seriously and I would include interrogation on any chance of it be gynae related and don't let them trot out the "she's too young" bollocks...

I had upper right quadrant pain and was in and out of hospital needing morphine. Ultrasound, mri, ct scan and bloods clear. Was told I would have to live with the bouts of extreme pain. Luckily for me a different passing doctor overheard and sent me for a HIDA scan to test the function of my gallbladder. Showed my gallbladder wasn't working and had it removed. Have been in perfect health ever since. NHS can be great but sometimes the opposite. It can feel like you are having to fight your corner. Keep pushing. I hope you get some answers soon. x

Morning, would you believe we’ve been here since 12 yesterday and haven’t actually spoken to a dr yet! Saw 2 doctors when she got blood taken but they were solely there to take blood, which took an hour Shock

She got a very thorough exam by senior nurse, the first person in all these weeks to look in her mouth & ears etc. She found a swollen red tonsil with puss, and swollen neck glands on that side. However DD doesn’t have a sore throat. Nurse said if she had a sore throat and fever she would diagnose glandular fever, as that would fit with the abdo pain - but it’s a mystery without those signs. She took a swab and we’ll get the results in a day or 2.

They didn’t say what bloods they were doing but I know they were testing different things to last time, was also told a dr was going to have another good look at the last US results, see if anything could have been missed.

I definitely feel like they are taking it more seriously this time. It’s hard as because she’s lying still she seems 100% fine. The senior nurse asked her to do a short walk, which she just about managed - then asked her to hop and jump. Poor DD looked like she was going to cry, there’s no way she could manage that.

When they decided to take my appendix (20 years ago mind!) it was on the basis that I couldn't hop or jump despite not having any other signs and symptoms.

You're in the right place, rounds will start soon and you can talk to the doctor then. Don't be afraid to ask exactly what they're testing for and why. And don't be afraid to tell them exactly how poorly she's been.Even though she seems OK ish lying there now.
