Informing emplyer of pregnancy - key info to include in letter? Does anyone have a template?

Just found out I'm pregnant with my first child and I need to inform my employer. I know I can leave this quite late, but because of the nature of my job I want a risk assessment/changes carried out straightaway.

What information do I need to include in my letter? So far I have thought of:
-I am approx xxx weeks pregnant (do I need to give a midwife/doctor's letter?)
-Expected due date of xxx
-Intention is to take full 52 weeks leave
-Would like to decide when to start mat leave later on (am only 4 and a bit weeks now)
-Need clarification on what annual leave I have to use before mat leave as I am due part way through a holiday year
-Please can you do a risk assessment!

Any thing else? Anything to leave out?

Would it be sensible to send a copy to my boss and one to HR (large company)?

Does anyone have a template letter?

I would just include the fact that you are pregnant, due date, and that you would like a risk assessment.
Maybe the bit aboiut annual leave if you wanted (or could leave that for a bit).
I would not include anything about how long you want to take or when you want to start mat leave at present.

You don't need to tell them your intentions regarding leave now. Just your due date. Hr will need your matb1 when you have it.

Four weeks is very early to tell them formally and to potentially have everyone know. Is there any way you could just speak verbally to a manager or HR?

You don't really need to put much in the letter now, because you won't know most of the information - due date (might change at scan), when to start leave,etc

Agree with ribena, very early to be writing formal letters. I would ask to speak to your manager - obviously don't know the nature of your job but do you need a risk assessment right now?

If you do need to tell them formally, then leave all the details about how long you plan to take/your leave, etc, til much much later. You may change your mind!

Thanks guys. I'm on holiday at the moment so it'll be a bit further on by the time I'm back. I was also going to request in my letter that as it is very early I want the information to be treated as strictly confidental.

I will write a very short letter then and leave out details about leave/holiday etc.

My job involves exposure to a wide range of toxic chemicals and also a lot of physical strain (heavy & repetitive lifting etc). I am worried about the safety of these whilst pg. Also I am very very tired at the moment and may well struggle to carry out my usual physical work anyway.

I have seen in the past my company making pregnant employees desk-based for the duration of their pregnancies so obviously the risks are considered important.

Does sound like early disclosure is important then, so I think a chat with your manager definitely needed .
