Accident at work - I'm the employer

My nanny slipped down the stairsa couple of says ago and bruised her bad quite badly. It is still sore today. She has been very good about it and is in no way blaming us, it was just one of those things.

But as an employer do I have any H&S duties to record it or do anything else? I just want to do the right thing. I work for a large employer and if I have an accident at work we have to fill in a form, I think so they can do some kind of risk assesment.

As a self employed doula, I have an accident/incident book- to record anything that happens, to protect myself and and the client. I'd have thought perhaps this isn't a legal requirement though but not sure.
A preschool for example, has a list of policies and would do a regular risk assessment for both staff and children. But not sure as you're a soul employer. I did a first aid endorsed by Ofsted so I think that's the reason I have to have an accident book. Sorry can't be of more help. I bet the HPA would have details or health and safety exec.
