Just feel like resigning

I really appreciate some advice on this.

I am in my 40s but don’t have much workplace experience due to long-term medical conditions. Despite this, i ve always been driven and completed both a BA and MA against the odds. I currently work two jobs—one during the day and another some evenings.

Recently, I had Covid and was off for three weeks. Due to my medical conditions, recovery took longer, and getting back to work has been tough. However, my manager hasn’t eased me back in at all. Instead, they’ve been piling on the pressure, even though I had unresolved IT issues that were preventing me from working properly. The IT team even contacted my manager about the problem, but he still brought up performance concerns.

On top of that, only one colleague privately messaged me to welcome me back, which made me feel a bit isolated.

I’m feeling really stressed by the pressure my manager is putting on me, to the point where I’m considering resigning. However, I don’t want to make a rash decision. Should I try addressing this with my manager or HR? Has anyone been in a similar situation, and how did you handle it?

I'm going to put to one side the fact that only one colleague welcomed you back - they aren't required to do so.

After three weeks off I wouldn't expect a phased return unless you actually said that was what you needed and explained why. Most people wouldn't require any special considerations and just because you have long term health issues doesn't mean that they should think you do. I say that as someone with severe disabilities. If you come back to work it is usually assumed that you are fit to do so.

HR are there to represent the employers interests - unions are there to support you, if you are a member.

What is not clear here is what you want? What is it you want to ask your manager/Hr for?

Did your GP sign you off and then indicate on the fit note that a phased return was required? If not, I can't see why you would expect it otherwise. If you have disclosed a disability to you employers and that would impact on your ability to return to work, that's a matter to discuss with them.

IT concerns are a completely separate matter and should not be conflated with your health issues. Nor should your non work related relationships with colleagues.
