I'm out sick from work and my line manager is part of the reason as I came to the realisation that some of her behaviour is bullying.

I followed the correct procedure and rang my line manager to phone in sick. I then emailed my medical cert to her. I also emailed it to HR.

I told line manager that I had sent to HR and that I had asked HR to contact me directly if I need anything else.

In my email to HR with the cert, I asked that if they need anything further, can HR contact me directly as I am finding it very diffulicult to work for line manager.

Line manager has emailed me a few days later asking for 'a chat'.

Do I need to engage with manager? Aside from her bullying behaviour, she is also a gossip and has spread incorrect information about 2 of my team members in the last month.

What should I do?

Unless you e raised a grievance/complaint about your line manager, they have the responsibility to manage your sickness. What’s the reason given on your fit note?

I haven't raised a grievance or complaint but did tell HR when sending in the cert that I am finding it very difficult to work for her and that's why I was requesting HR to contact me rather than manager.

The cert had 'medical illness' on it. I asked for this because of manager gossiping, I don't trust her. I told HR if they want Further details on the illness they can let me know.

I had a team member out for 3 weeks earlier this year for a 'medical illness' and I wasn't required to ring him 'for a chat' during that time.

hmm sorry OP, unless there’s a formally documented reason you don’t want interaction with your manager, she will still be responsible for managing your sickness, even if you have asked HR to bypass her..

Sloppy management not to be in contact for 3 weeks, unless you agreed at the beginning that was how much it was going to take.

It isn’t HR’s role to manage you. I don’t know what you expect them to do with “medical illness”.

What are you off with? How long will you be off? How can the issues with your manager be resolved? You need to take a more active part in managing this. If you want HR to do something (allocate a new temporary manager?) you’ll need to actually ask for it and give them the info they need.
