Depends on the disability and the context of the employer. For instance if they do home/hybrid working they're probably going to be more amenable to it.
As poster says upthread, it's not just a case of saying what you want and getting it.
Most reasonable adjustments are free/inexpensive/easy to implement, but not all. Home/remote working is a little contentious at the moment as so many employers are trying to get people back to offices regardless.
The inclusion passport (we call it reasonable adjustment passport) is really just for recording what's been agreed/ not agreed in my experience, not a route to be given whatever you want (or need, unfortunately).
If the adjustment can't be easily agreed between all parties an occ health referral is the way forward, but again that's not a guarantee - OH give advice not instructions. I've dealt with a lot of health related home working requests over the last couple of years and OH would always say it's a contractual matter between employer/employee.
For hidden disabilities I always had to ask the employee to get evidence from their own medical team in order to get HW contracts agreed by the organisation as an adjustment (there were many unreasonable hoops as they didn't want to give new HW contracts).
