Disclosure re: social services involvement

Hi all, gone NC for this. I’ve just been offered a job working with teenagers and need to fill in an Employee Details form, which asks the following two questions:

Have you ever been known to any Adult's or Children’s Services department or Police as being a risk or potential risk to children or vulnerable adults?

Have you been the subject of any investigation by any organisation or body due to concerns about your behaviour towards children or vulnerable adults?

Three years ago, a report was made to Social Services for one of my children by their school after they made comments about their eating/access to food at home (they have an eating disorder). Social Services visited and found no concerns and no need for follow up. Do I need to answer YES to the above questions (and which ones)? Many thanks.

Oh thats a tricky one. It says behaviour rather than care. But I would say talking to them about it would be a positive thing, I would expect them to be supportive that your child had an eating disorder. I would say ring them up and tell them about it, to be honest if they reacted negatively then you wouldn't want to work for them anyway. The safeguarding wasn't upheld so there is no issue here. Loads of families have social services assessments we did ourselves.

No you would not need to declare this unless your children had a child protection plan.
I queried the same as I rang NHS direct about a injury my dd had and was told to take her to the gp in the morning.
I ended up taking her to an out of hours doctor instead.
Social services rang me 2 days later saying that they had had a referral because I had not taken my dd to the gp. When it was explained that she had seen a medical professional it was marked down as ' no further action'.
I was told this didn't need to be included in a similar disclosure.
