Any NHS managers or NHS HR people here?

I'm in the NHS, actively looking for new roles. In the Trust I work for, it's the line manager (rather than HR Dept) who do references.
A few months back, I was put on an informal action plan (due to performance issues, I'd been making mistakes). I didn't disclose at the time I was struggling with a mental health issue which affected me at work.
When I disclosed this (in confidence to my line manager), he took me off the informal action plan as he felt staying on it would be unfairly punitive to me when I'm struggling health-wise. When taking me off the plan, he referred me to Occupational Health and also the staff service that helps employees in my situation.
I've also been to my GP and started some meds.
My question is, if I apply for new jobs (both inside and outside of the Trust), would my mental health issues and my brief spell on the informal plan be mentioned in a reference? My manager has stressed the point that he doesn't feel disciplining me or keeping me on that plan would have been helpful, but I think he only changed to this belief when I disclosed my mental health issue. Otherwise I'd probably be heading down the disciplinary route.
Currently applying for new jobs (need something without antisocial hours/shift patterns), ideally school hours to fit around my 3 children. Just feel worried about this issue showing up on any reference.

There’s no way it will come up. You’ll be fine. Most trusts now just say “yes they have worked for us for X years” and that’s all. Don’t worry!
