Disciplinary at work

I'd appreciate advice on what might happen and how to deal with it.
I made a mistake at work, twice. The first time they let it go, then I stupidly did it again. It is a v high pressure situation and it's easy to make a mistake, but it can be v serious to health. (It wasnt, either time, on both occasions someone noticed the error before it became a problem. )
I am undergoing a disciplinary investigation.

All the advice I am reading tells me how to contest it and advises collating evidence etc.
But I did make the mistakes.
So I can't argue that I didn't

So what will happen? Will I just get told off, like a school kid? What am I expected to say?
Obviously I'll apologise and tell them what I suggest so it doesn't happen again.

But other people have had disciplinaries for the same thing, and I have suggested changes in the past to stop it happening, but been told no.

So I dont know how to approach this.
Does anyone have any advice please?

If the same "event/ mistake" is reoccurring this surely suggests that the process surrounding it needs to be reviewed.

The fact that there are others who have also previously made the same mistake as you, and that this is the second time you have unfortunately erred, this is very clear evidence to support your case that it's not all down to "human error"

Why have they said they can't implement suggestions to mitigate the risks? Seems very short-sighted.

Some processes have been put in place, and tightened every time a mistake is made
Then, to try to stop it happening again, they decided to make it a disciplinary matter for anyone in the future. So far I am the second person to go through it.
I was told the last idea that someone suggested would take too much time and not be practical. I thought it was a good idea. But I dint make the final decision.

I'm just looking for some ideas on how I'm supposed to behave/react at the meeting. I wont pretend I didnt do it, cos everyoneknows I did. I can be apologetic and promise it wont happen again, but in all honesty i cant guarantee that. It's a skill i have no natural aptitude at, hardly any training and not really part of my job.
But due to my position I am expected to do it sometimes if the person whose job it is is off sick.

I think you say exactly that.

I made a mistake. I am sorry.

However in mitigation I have not had adequate training and it also does not form part of my job description. A recent risk assessment made suggestions which would have been helpful for these situations but was not acted upon.

Thanks, when you put it like that it sounds professional. In my mind it sounds like moaning. If I use your words it will sound ok.
