Help i am struggling to complete this application form!

It is the part for additional info,it says explain your skills,abilities ,experience and achievements to date which make you a suitable candidate for this post.
I really cannot 'sell 'myselfand the form has to be returned by wed!
The job is for a Health care support assistant.

Only trouble is that my workplace include days off in sickness so i couldn't just put 10 days.

Put 2 weeks then? Or will new work assume 14 days is 2 weeks? I wouldn't want them to think it was longer than it was, but obviously if new place also counts days off that's fine, they'll know what you mean!

on various paperwork I fill in for work, we have to complete two parts of what appears to be the same thing - we record length of absence (in this case two weeks) followed by number of 'rota days' - which is the number of days you would have been expected to be in work, but were off sick. The second number is always lower than the first and for staff on part-time contracts "two weeks" might only be four working days. (SWIM?)
Do you have a way of getting something like this into the bit of the form where required -
"length of absence : 2 weeks (..x.. working days)" or something? hope it makes sense??

I think i will put something like

"2 weeks sick leave including days off following one period of absence."

Is that ok?

Hi sorry just seen this,
well gals you are going to really hate me for this but ....i do have an interview(only because of all the help you all gave me) but i'm probably not going to go now
Initially i applied for the job as i was worried about the security of my current job(long term future to be decided) as they were asking staff to volunteer to relocate or be relocated by them.I was unsure if i would cope with the 'move' to another area and doubtful if i would like the group i would be working with.Anyway i had to make a really difficult decision last weekend whether to move or wait and be moved and i went and did a couple of 'trial' shifts to see if i would like it,which i have so far.So on thursday i decided to relocate there.
I work with adults with severe and profound learning disabilities and the new group i am working with have severe challenging behaviours,which actually is quite a challenge for me and suprisingly so far i have enjoyed it.
Anyway this made me question whether i really wanted to return back to elderly care having done 11 years previously ,and i really feel that although the job would probably pay the same and offer the same pension plan etc ,it would also offer more security but i am really enjoying working within the learning disabilities area,which i have done now for 4 years.So i guess i will have to wait and see what the long term future of our company is.
The interview for the position is also 45 minutes long ,i can't believe it is so long after everything they wanted to know on the application,what else could they ask?
I would hate to go for an interview and waste anyones time.
I feel 80% that i am doing the right thing ,the occasional element of doubt creeps in ,where i think to myself "What if i regret not taking the opportunity?" ,as it is a secure type of company.
I have made myself quite ill with anxiety attacks trying to decide to be honest.

Anyway i am so very grateful to all of you for helping a 'cyber stranger'and for your time and effort,i cannot thank you enough.
I hope i haven't upset anybody
I hope and pray that i am making the right decision .

Don't be silly pelvic - you've got to do what's right for you. Is it worth going to the interview anyway just to see? As you are no longer 'needing' the job there will be less pressure on you? Just a thought. It will probably still be stressful though! If you decide not to go, hang on to copies of your application, I am sure it will be useful for any jobs you might apply for in the future. Good luck with whatever you decide .

Pelvic congrats on getting interview!

45 minutes is perfectly normal, that's not excessive at all, and gives you good opportunity to sell yourself.

Don't worry about upsetting anyone if you don't go! I would say have a think about going anyway, you don't have to take it if you get offered the job but it might be useful to reinforce your decision to stay and get rid of the nagging doubt.

Have a think about it anyway

i'm inclined to think the same as pink and flowery and go to the interview for the "fun" (?? i know) of it and just to be nosey. Having a go can do wonders for your self-confidence!

I'm glad to hear you're enjoying the re-launch of your job...and it does your employers no harm to get the odd ref request, keeps them on their toes
