How to you ask for a promotion?

I'm in the private sector, so job roles, titles and salaries are basically a moveable feast. Based on the work I am currently doing compared to people a level above me (first level managers), and people on my same job title (senior officers), I think I've got a good argument to ask to be considered to move up to the level above. I'm doing far more managerial level work than my fellow senior officers, I'm the only one with line management responsibilities, I run big projects on my own etc. In our firm they often have people 'working towards' a promotion within a year or similar.

I'm trying to see this as being confident and say that I am currently doing a good job, that there is scope to be doing even more, and I'd like them to seriously consider looking to move me up a level. But despite being nearly 40, I have never done this before and I don't actually know how to go about doing it and putting across a good case.

Any hints or tips?
