With reference to your letter dated xxx and our telephone conversation on Monday 2nd July, I am writing to confirm that I will be returning to work for 22.5 hours per week.

I would like to request that these hours are allocated as 8.15am to 4.15 pm Tuesday to Thursday dn llow for 30 minute break each day.

I understand that the localities have now been merged and whereas previously I would work a group of local (Is-there-a-word-missing-here?) I now will work with any practice in the PCT.

I accept that I will have to travel to meet the needs of the Trust and would appreciate clarification regarding the travel arrangements as I was previously advised that travel to other locations from my base at xxx would be within the above-mentioned hours of work.

*(I-wouldn't-include-this-in-this-letter-but-would-deal-with-it-as-it-comes-up)

I would like to take this opportunity to inform you in writing that I will do my utmost to ensure that all my child's appointments with regards to his/her additional needs are scheduled where possible on those days when I am not working. However I may require flexibility with clinic-based appointments.

You probably want to also include the date with effect from which you will be returning.

I would have thought you should deal with that on an ad-hoc basis and would be easier not to put in writing?

but I don't know what your job's like so might be wrong

Twig's version is v good...

My contract stipulates my "base" as XXX (fill in the location of your office)"

Subtlely making the point that you are NOT going to be travelling miles away at the start and end of the day.
