CV for p/t job. What needs to go in it?

I'm applying for a p/t job, just waitressing, but they all want a CV. Whatever happened to application forms?

I haven't done a CV for about 10 years, and don't know what to put.

I know I shouldn't put in my qualifications, as they're totally irrelevant, and do I put in all jobs I've had, or just a select few?

I really need a job, and don't want to oversell myself, but obviously don't want to undersell myself too.

i go into detail with past three jobs the put something like:
previous: then list the companies and dates you worked there.
though my first job was in 1986!

If you've got any qualifications, then these should go in. Contact details, including e-mail and mobile.

for a waitress job they will be looking for
personal info
age/name /address / phone / email/ what hours you are available for work/ and 2-3 sentence about what kind of peron you are. (friendly, honest, reliable, knowledgeable of food hygene practices)

your experience
(Have you been a waitress before? If not, please put any customer-facing experience you have.)

references
(who can they contact? choose someone who is easy reach by phone and who will respond right away if they contact by letter or email).

Also write a short cover letter telling them what a good waitress you will be and close the letter with something complimentary about the resturant.

I just typed out a CV for me as the last one went missing. I really struggled so i done a search under job description for xxxxxxxx. You will get loads of ideas, maybe say you done some waitressing years ago and blag it if you can.

SOme ideas

As a waiter or waitress you serve food and drink to diners at their table. Your tasks would vary depending on the establishment you work in, but all include taking orders and serving food and drink. Making customers feel welcome and comfortable is an essential part of the job. To do this work you should have a welcoming, friendly attitude, a good memory, a high standard of personal hygiene and should be well turned out. You also need excellent communication skills and basic mathematical skills for calculating bills.

Check patrons' identification in order to ensure that they meet minimum age requirements for consumption of alcoholic beverages.
Check with customers to ensure that they are enjoying their meals and take action to correct any problems.
Escort customers to their tables.
Explain how various menu items are prepared, describing ingredients and cooking methods.
Inform customers of daily specials.
Prepare checks that itemize and total meal costs and sales taxes.
Present menus to patrons and answer questions about menu items, making recommendations upon request.
Remove dishes and glasses from tables or counters, and take them to kitchen for cleaning.
Serve food and/or beverages to patrons; prepare and serve specialty dishes at tables as required.
Stock service areas with supplies such as coffee, food, tableware, and linens.
