Flexible working

I'm on mat leave which is due to finish in a few months. My line manager agreed for me to work 3 days per week verbally, on my return, this was agreed months ago and has since moved to another role. My new line manager is arguing that I need to follow a 'proper process' ie fill in a form and she's inferred that she will decline 3 days. My argument is that I would have completed a form months ago if I was sent the form but that contractual changes are also made verbally so it's been agreed. Any changes would need to be completed via a formal change of hours process.

I have been in touch with my old manager (works at the same company still) and she's said that she shared details with my new manager and that it's out of her hands now.

WTF do I do?

I would fill in the form. There is a list of the 'allowable' reasons your employer can give for refusing your request, but you can appeal if that happens. But is might be as well to be aware of these allowable reasons when you are applying. this government webpage explains it.

You fill in the form. If you've still got "a few months" left then there's plenty of time.
