My dd was sent this survey to complete via her school. They were asked about their sexual orientation and had to choose from a list and also their 'gender modality' where they have provided a helpful explanation of what transgender means. Whilst I am certain that the underlying intent of the survey is a good one - helping to improve children's mental health. I can't help but think asking 13 year old children about these issues is completely inappropriate. Any ideas on how I should approach a complaint email ?

Agreed, not appropriate at all, extremely intrusive and the fact it's opt out not opt in is unethical.

Where's the evidence introducing these concepts is not harmful to mental health in children? There is none.

I'd be asking why teachers want to know the serial orientation of my child and report it as a safeguarding issue.

There’s an agenda here. The questions are not clear- ‘girl including transgirl’ that’d be a boy then?
This must be a safeguarding issue?

I imagine myself as a 13 year old, trying to make sense of it, perhaps trying to write the ‘right’ response. I just don’t get why it’s necessary
I’d put it firmly back on the school, not some random researchers!
Am I glad my kids aren’t at school anymore.
Although college and uni have their moments

I wonder how many parents simply wouldn't know about this? I can't find anything about parental consent after a brief peruse of their website. The support page says this "We want to make young people’s wellbeing everybody’s business, and we hope you do, too!"

Er, no, that's creepy and inappropriate.

Lots I should think.... If they didn't see or thoroughly read the opt-out email then their children's future health records can be accessed for... life.

There's lots on this on the linked thread. They appear to believe that they meet a special clause in UK GDPR that allows them to do this without opt-in parental permission.

It's shocking frankly. The questions themselves, which position gender identity belief as if it's fact, and the approach that the university is taking to do the research.

Edited to add: this is going to tens of thousands of children across parts of the UK. I really hope this unethical survey gets the exposure it deserves.
