Had reason to look NICE guidance up today. Note These are not new guidelines - but why, just why do they talk about urinary catheterisation by gender ?

From a panel of experts too.

Because it’s referring to what type and size of catheter to use.

TheShoesa · 10/03/2020 17:30

It's a document for use in a medical setting. They should use the word sex.
Might be worth an email if you've got the time?

heather12911 · 10/03/2020 17:31

Women need different sized catheter's to men as they have a shorter urethra

GrinitchSpinach · 10/03/2020 17:31

Surely the size and type to use are determined by the patient’s biological sex rather than internally-felt gender?

Al1Langdownthecleghole · 10/03/2020 17:41

Yes, Theshoesa and Grinitch. The requirement is based on biology, not feelz.

Of all the places...

And yes, I will be responding in my professional capacity.

This is the Royal College of Nursing’s guidance for the urological care of transgender patients. This is entirety of what they have to say on catheterising transgender patients.

”Trans women can be catheterised as with other women, although do bear in mind that the physiological markers may be less evident than in other women.

Trans men will need to be catheterised based on their genital presentation. Trans men with a phalloplasty can be catheterised, as with other men.“ (Page 8)

www.rcn.org.uk/-/media/royal-college-of-nursing/documents/publications/2017/march/pub-005844.pdf

It is clearly incredibly poor and ill-informed advice which helps neither the patient nor the nurses. The guidance doesn’t for example, acknowledge that most trans people who are of the male sex retain a penis and therefore could not possibly be catheterised as with other women.

We need to keep chipping away at this kind of thing.

Am speechless chicken. That is an exceptional level of stupid.

^ Jesus Fucking Christ the Stupid.

Good to see the RCN upholding their commitment to the feelz based medicine.

ThinEndoftheWedge · 10/03/2020 18:59

RCN - would be hilarious advice if it wasn’t so serious.

Transmen need to be catheterised based on genital presentation - but transwomen don’t need to - what the fuck?

And transwomen’s physiological markers - (mostly penis) - may be less evident. RCN - please revise your anatomy and physiology or close down.

I'm not even slightly medical (although I have been catheterised) - but surely if someone's had any kind of surgery involving their urethra, you're going to want to be just a tad more careful and aware than someone with un-mucked about with genitalia. It would seem to me that things might be more fragile, or not the lengths you're expecting, or possibly in slightly different places/angles.

In any case, catheterising your average transwoman is not going to be the same experience as a woman, what with the penis and all.

Al1Langdownthecleghole · 10/03/2020 19:52

Not to mention the prostate gland which might not have got the message about gender identity.

I dunno. I think it may be somewhat different....
