the only people I've heard ask this were rather sheltered, and religiously educated in less rigorous countries.

A group of VI formers getting ready to present their 'health campaign' for their BTEC Level 3.
One group had chosen contraception to talk to year 8 about. They had to research various methods and one girl asked about the diaphragm needing to remain in place after sex, "what happens if you want to go to the toilet?"
They were fine with putting condoms on bananas, discussing STDs yet one of them didn't realise you pee out of a different hole.
