Ruptured dermoid ovarian cyst

Hi all,
Looking for some advice. Had an ultrasound in February which showed a complex ovarian cyst measuring just under 5cm with solid matter inside. I’ve had two surgeries before for large dermoid cysts and a mucinous cystadenoma (all on the right ovary. In the last ultrasound, my right ovary was double the size of the left one due to the cyst stretching it.

I’ve been referred to gynaecology but heard nothing and normally experience slight pelvic pain and lower backache. For the last two weeks, I’ve been having intermittent spotting and brown/black discharge.

I had intercourse last night and during it, suddenly had a sharp stabbing pain on my right side so we stopped. Since then, I’ve been having really bad pain and my stomach has swelled.

I ended up booking a private ultrasound for today. The ultrasound showed the cyst and ovary have shrunk considerably but the solid matter inside had more than doubled. It also showed an echogenic area. Due to the pain/swelling and bleeding, the sonographer believes it has ruptured.

Do I need to do anything from now on? I’ve looked online and it’s mostly about simple ovarian cysts rupturing, not complex. Also any recommendations for pain relief if anyone has experienced this would be greatly appreciated. I’ve been taking naproxen but it isn’t helping at all.

Didn’t the sonography do or say anything about what should happen next? I find that astonishing, as it has serious implications.

I had a large dermoid cyst. I was advised they rarely rupture but can become affected by torsion (twisting), which is a medical emergency.

In the case rupture, the oily or fatty contents can spill into the abdominal cavity, which can cause chemical peritonitis (inflammation of the lining of the abdomen) — this is painful and can be serious.

The symptoms of rupture can include sudden severe abdominal pain, nausea/vomiting, fever, and tenderness. Treatment typically requires urgent surgical removal and cleaning of the abdominal cavity to prevent complications.

You should be treating this as urgent and get medical attention.

When I was waiting for surgery on my dermoid cyst I was told in no uncertain terms to go to A and E immediately if I experienced what you describe. Hey yourself there asap.
