Fecal Loading, Slow Transit & Mild Prolapse - Help!

Hello,
I'm posting here in the hope that others may have similar experiences and can share what has worked for them as I'm overwhelmed and nothing is helping.
I have suffered from constipation for most of my adult life (not helped by laxative abuse in early 20's due to eating disorder which is now dealt with). About 15 years ago, I had a test that confirmed slow transit and was told to take Movicol forever which seemed to work for a long time.
About 5 years ago I noticed a mass in my right side - long and sausage shaped. Was scanned and told I had severe fecal loading and to use more Movicol to clear it but even with increased Movicol I can feel this area full all the time. I had another ultrasound a few weeks back to check ovaries and again, they noted high fecal load in ascending colon which was a surprise to me because I am actually going to the loo!
I eat very well, tons of fruit and veg, lots of insoluble fiber in the form of bran and flax etc. I exercise plenty - 10K steps per day, yoga, pilates etc
I've recently been told by a physio that I have a stage 1 vaginal prolapse (probably due to long, difficult labor 7 years ago).
I have soft stool from Movicol but rarely have the urge to go until I end up with diarhea. If I strain (which I find hard as feels like my vagina is bulging) I can pass a thin soft stool which I read can be due to pelvic floor.
I had abdominal surgery in 2020 to remove a 6 inch dermoid cyst so I expect I'm also full adhesions which are likely not helping.
I feel pulled in so many directions and not sure how I can help myself. In addition, the fact that I find it harder and harder to go to the loo in spite of increased Movicol and fiber makes me worry about the worst case scenarios.
does anyone have experience with slow transit + pelvic floor problems and what if anything helped?

What about a kegel8 trainer to help with the prolapse? Really helped with my whole pelvic floor. You may have a retrocele as well, so the stool is bulging into your vaginal wall

Thank you for sharing. Would a retrocele be felt during an iternal exam by physio?

If you took something like bisocodyl (and drank lots of water) it would help with bowel motility. It is less predictable and more likely to cause cramping though. I’m wondering if you could ask your GP for a prescription for Domperidone. It also increases bowel motility. (But can have side effects).

It could be that you are having too much fibre for your bowel to cope with. With STC, sometimes movicol is not enough. It makes the stool soft but there's no impetus for the bowel and rectum to clear. It might be worth trialling a low residue diet and an irritant laxative to see if that shifts the loading.

Seeing physio is a good idea. Do you see a dietician or a gastro? Seeing a gastro interested in STC could make all the difference in your management.

This is interesting! I eat tons of fibre. I'm seeing a gastro specialist next Tuesday so will ask about this.

A good pelvic floor trainer may help. Reflexology can be wonderful for helping digestion too.
