Prolapse post hysterectomy

About 3 years ago elderly DM had what we believed to be a vaginal hysterectomy as well as having her prolapsed bladder stitched back up. All was fine. Then a few months ago she experienced another prolapse again, is in a great deal of pain etc. She went back to the (same) consultant who told her it was her prolapsed uterus...DM is the sort who doesn't like to ask questions or cause a fuss so meekly agreed to come back in a few weeks to discuss the plan of action.

I'm not being daft am I....that doesn't sound right?! Or have I missed something?? Would a hysterectomy not involve the full removal of the uterus?! Again, at the time she didnt want to ask too many questions and it was during covid so she was in by herself....

I am going with her to the next appointment...Ive got no problem with asking questions!

Is it likely that she misunderstood the first procedure?
Did she have stitches in her abdomen? Vaginally assisted hysterectomy still have tools/scope etc that go in through incisions in the abdomen so there are a couple of sutures in each incision.

I think thats a strong possibility to be honest. Although she showed me some notes from before the last procedure setting out what was planned which definitely said vaginal hysterectomy....nothing from afterwards though. Really odd.

I dont think there were any incisions on her abdomen, no.
