The trials and confusions of Pcos- absent period followed by blood clot

Hi all, would really appreciate any help or advice regarding this issue.

I’ve had PCO turned Pcos ( is that even possible) since I was 22/23 ( diagnosed in 2018) . Period has always been irregular but would have a few years where they’d be like clockwork. Only symptom I’ve ever had from textbook pcos is the absent periods, but now as I’ve gotten older, and especially in the last two years I’ve put on a SIGNIFICANT amount of weight in 2020 which stopped my periods all together. I’ve been having a “period” every 3-4 months since then, but not characteristic of my normal periods at all, in that there are no cramps, it’s not heavy and it normally starts off with stringy brown blood and lasts for 9 -12 days, barely filling a pad .

So, yesterday I was on my feet most of the day, and had slight lower back ache ( put it down to being on my feet the whole day). I went toilet and noticed bright reddish/brownish blood. Thought ok my period may be coming, few hours later I felt a gush almost typical of the ones I felt when I used to have heavier flows a few years ago. Saw a medium sized blood clot that was quite long in size. Thought my period might finally be coming, but then after that had absolutely nothing. Clear pad today as if yesterdays madness didn’t happen.

hate not knowing what’s going on in my body. Now I’m wondering if it’s old blood just coming out ? I did have intercourse the day before so thinking if that triggered the release of this blood? Happy to post a pic if anyone wants ( bit TMI I know)

Would love to have any advice or if anyone’s had something similar happen?

Yes, I think the intercourse and associated lubrication, dislodged the blood and triggered movement.

Do you exercise? Any vigorous walking, hiking, climbing, movement in the pelvic area..... I know this helps to trigger my period/release.

I think (no expertise) the blood is building up but not being released, it's the progestin (sp) hormone that helps to push everything out. This may be out of whack? Could you be peri menopausal? Are you on any contraceptive pill?

I think if blood doesn't dislodge and remains in the uterus, over time this can become risky and is something that should be avoided, in the long term. Unshed blood is potentially dangerous.

Could you have shed the lining - decidual cast - rather than it being a clot ?

@CreationNat1on thank you for the response! Yes that’s what I thought. I’m normally not that active but the day this happened, I was on my feet most of the day cooking/cleaning/baking as I had an event. Possibly could have triggered it but was confused as I’ve had no spotting/period like blood after the clot.

I’m not on any contraceptive pills. Hope I’m not peri-menopausal as I’m 28, but I guess with PCos it may be a possibility. You’re right that not shedding blood every month isn’t good, I’ve been trying natural remedies to lose weight in hopes of regulating my period but may now contact gp to be put on progesterone tablets to get a monthly bleed. I would like to start thinking of ttc but with my weight, pcos and out of whack periods it may be a long road.

@DustyLee123 I did think this after some Googling, but the decidual cast almost looks like a red/pale ish large blob ( some compare it to liver) , whereas what I had was more brown/dark reddish jelly like clots with dark red blood.

My DD has PCOS and was happy not to have periods, however her GP advised her to take the tablets for a period every 3 months.

@DustyLee123 yes from a health perspective i guess that’s wise. I think I will be doing the same once I get a gp appointment.
