PCOS - getting worse?

I've had a PCOS diagnosis for three years now. Three years ago I had "only" one polycystic ovary and no symptoms other than acne, although blood tests conclusively showed I have it. Things seem to be ramping up, though.

  1. Over the last 1-2 years, I've rapidly gained a lot of weight as my cravings have been out of control (no apparent external reason such as stress, new medication, lifestyle changes, etc.). I was a stable 21 BMI since teens until a year or two ago... now I'm over 27 and I've given up managing it with exercise and diet as I can't seem to turn a corner.

Pending further blood tests, Obgyn said he'd put me on metformin and refer me to a dietician, and if this combo doesn't work, try Ozempic. Does anyone have experience with metformin and/or dieticians, and did it make a meaningful change? Does it help with the cravings? How long am I expected to try this until I should ask for Ozempic?

  1. Both of my ovaries are polycystic now (did an ultrasound). Obgyn said this doesn't necessarily mean anything but I wonder now if it's representative of my broader condition getting worse?

Periods and ovulation still seem normal. I am 29 and we want to start ttc soon, but I'd rather get my weight under control first as I worry that I never will otherwise...

Hi, I also have PCOS and recently started metformin 2000mg a day. It did upset my stomach for a while but not as terrible as I had a read about. I was just like you, gained weight really quickly. With the metformin I’m not dieting because honestly I’ve done that my whole life and I’m just sick of it, just eating sensibly and my cravings are now under control since metformin, I just don’t have cravings anymore and seem full from eating much less. I am now losing weight slow and steady but hey at least it’s going in the right direction!
I didn’t have any tests regarding insulin resistance to confirm, the consultant was happy to prescribe based on the diagnosis alone.

after a month on metformin I did fall pregnant, was sadly a chemical but I’m hoping if it made such a difference so quickly it won’t be long before I see another BFP

And really good to hear about the cravings. That is my main issue; I know what's "good" for me but I just feel like I NEED a burrito right now!

I know exactly what you mean ! Haha
sometimes I get short lived cravings, like I will really want chocolate so get some, where I would normally finish off the whole pack I’ll have a few bits and then just feel done with it and don’t feel the need to have anymore … if that makes any sense

@Crazyhope I've been on it a week now and that makes complete sense - it's easier to stop when I'm full rather than just keep going for the sake of it... thanks again :)
