Pelvic pain at night lying down?

I've had pelvic pain on and off for about seven years now and had some investigations, smear, internal ultrasound. Nothing found. At one time the pain only came when I stood up for a long time and eased when I sat down. It's a heavy, tight, congested pain.

About three years ago I started to get pain when lying in bed at night. All across my lower abdomen, relieved by urinating, waking me four or five times a night. I decided from googling that it was probably bladder sensitivity, possibly connected to peri menopause (I'm 50) and I asked my doctor for HRT and amitriptylene (I read this was good for numbing the signals from overactive bladder). This combo worked like a dream and for a couple of years I was able to sleep soundly all night, no getting up to pee, no pelvic pain.

Over the last few months the pain has started to come back. I'm getting up four or five times a night again, my bladder/lower pelvis is so sore when I wake up. Peeing relieves it but I'm left feeling bruised for some time after getting up; I have no burning or pain when peeing. During the day when I'm upright I'm fine.

I spoke to my gp a few weeks ago on the phone for an HRT check and asked for testosterone as I'm feeling pretty crappy with no energy or drive. She sent me for a blood test to check my estrogen levels instead as she thought I might be low - I take Novofem tablets. I have not heard back from the gp yet although the blood test was weeks ago. Suppose I should chase this but my surgery is such a bloody faff.

I've tried reducing coffees to mornings only and drinking more other liquids like redbush tea and water, doesn't make much difference.

Does anyone have any suggestions as to what this might be? Thanks

Same here.
My physician (internationnally welll known[ told me it was because my internal obturator muscles were too tighten, he prescribed kinesytherapy (relaxing exercises[, i have to do ito see if it works.

Hi, thanks for your reply. I'm still suffering, have tried changing my food and drink in line with diets for interstitial cystitis in case it was that but saw no difference.

I have MS and my nurse suggested it could be tightness and internal spasm of some muscles, so your post makes sense to me. I'm going to google some relaxation exercises for the pelvis.

My pain is centred around a spot in my lower right pelvis, it feels like it's pinching, but it radiates all across my lower belly.
