PCOS question

Hello, just after some info regarding PCOS. Will try and keep it concise, always had regular periods 28 days apart and seven days long and painful and heavy. I have two children, first emergency c section, second elective. After my youngest was born (he’s four now) my periods became 14 days long (still regular in frequency) saw numerous doctors and had a few ultrasounds over three year, no problems ever found. They tried to do a hysteroscopy under local anaesthetic but they couldn’t (I have retroverted uterus and this apparently makes it difficult) so I went on waiting list to have it under general anaesthetic. In the meantime my periods suddenly reverted back to normal so after consultation with them I decided to go on what they called a watchful wait, basically they wouldn’t discharge me but I was taken off the waiting list for hysteroscopy. Anyway I’m about six months down the line from that, periods have started creeping up in length, last one was early, latest one is five days late (this is unheard of for me as the frequency is regular it’s just the duration). I have terrible bloating and back pain and feel awful. I was worried and felt fob Ed off by my gp so I booked a private ultrasound. Sonographer was great, she showed me that my ovaries are mildly polycystic and I’ve had a cyst rupture and bleed she could also see that I had been ovulating from both ovaries and had already ovulated this month and the lining was nice and thick so should have expected a period. I haven’t had unprotected sex, we use condoms and I know they aren’t 100 per cent but you usually know if there’s been an accident, I’ve had one before. I took a pregnancy test a few days ago that said it could be taken from the day the period was due and it was negative (three days late at this point). Sonographer said it sound unlikely I’d be pregnant but that it was early to take the test and to retake in two weeks if my period hasn’t shown up. It’s confusing me that I’ve definitely ovulated but my period hasn’t come but she can see a thick lining? I don’t really understand it as I don’t know much about PCOS and google doesn’t yield an answer as to why the period hasn’t come if you’ve definitely ovulated?

I suppose the question is am I ovulating later in the cycle now and so I should expect this more often?

You may not have infact ovulated.

It is quite possible to have periods without ovulating and PCOS makes that scenario even more likely. Polycystic ovaries are actually cystic follicles; some of these release hormones whilst others are dormant. One may contain an egg.

What sort of doctor are you currently seeing; is this a consultant gynaecologist?.

Have blood tests been done to see what your current hormone levels are like. They should really be looking at your LH level in comparison to your FSH.

Periods should not be very painful nor very heavy; a common cause of both can be endometriosis.

Lots of women have mildly cystic ovaries on scan but that isn’t PCOS. The syndrome normally causes infrequent or absent periods. Along with other markers.

Regular but painful and prolonged periods however can be a sign of other problems like endometriosis so I’d be asking for a gynae referral.
In the meantime could you consider a mirena coil or even the combined pill to regulate your periods and make them lighter?
