IUD perforation - did the doctors miss it?

I had a Kyleena IUD fitted in September last year. The fitting was very painful, even with gas & air, local anaesthetic and dialators (not had a baby, apparently my cervix is v tiny). Two days later I am in such pain I am finding it hard to walk, so I head back to the clinic to check all is ok. Nurse couldn't see the strings, so sent me off for a scan. After lots of usual NHS waiting I have a vaginal ultrasound and the doctor says it's in the correct place and the reason they can't see the strings is because I am probably all swollen still from the insertion. It's important to note at this point the guy who did the ultrasound seemed very inconvenienced by this and he showed me the screen of the ultrasound pointing at a speck of slightly lighter coloured bit of screen saying 'that's it there'. Didn't come out of it feeling 100% satisfied that everything was ok as there was still no explanation as to why I was in such pain.

Was called back to my GP for my 6-week check post insertion and again, GP couldn't see the strings. For the previous 6 weeks I had also been having odd shooting-style pains down the left hand side of my vagina every now and again, and just thought this was normal with a coil. GP sent me back off for a scan again just to be sure. Vaginal ultrasound showed no coil in uterus. Was sent for an x-ray, coil was located but has perforated my uterus and is currently sat in my pelvis on the left-hand side (where I have been experiencing the pain). GP said likelihood is that it perforated when it was inserted.

I am now waiting for my referral to have it taken out surgically.

My question to you is - does anyone think that when I went back 2 days after my insertion, that the coil had in fact already perforated, but the doctor who did the ultrasound either missed it or was too bothered about getting home so he just said it was all fine? Am I within my rights to have a second opinion on that scan, if it had been saved? TIA
