Contemplating bariatric surgery.... advice?

Hi as the title states I am contemplating a gastric sleeve. I am a serial yo yo dieter and my metabolism is fucked. I have pcos and have tried Xenical, saxenda etc- successful until I stop using and the weight piles back on.

I’ve seen an endocrinologist and he said that, in his opinion, sometimes surgery is the only viable option. I’d be paying for it myself as I don’t quite meet the nhs criteria yet.

I’m miserable- I don’t lose weight unless I starve myself ie 600 calories a day and then I’m even more unhappy. I’m worried about my health in the long term, but then the prospect of surgery scares me too.

Any advice from those who have been there before?

@Seriouslyconfused3 here’s some photos for you! The grey T-shirt I’m wearing is a size 26 from ASOS, I just like to wear it round the house tbh.

I was circa 25 stone, I’m 13 stone 12 5ft9. I had health issues due to weight such as idiopathic intracranial hypertension. Severely addicted to sugar couldn’t lose weight. Just couldn’t didn’t matter what I tried I always went back to chocolate and would just consume so much. Also had sleep apnea. Had surgery on the NHS, really quick from doctors referral to surgery, 11 months in total. Which is very quick for the entire process. If I hadn’t have been able to use the NHS I would have gone to Dr Tamer Abdelbaki in Egypt. I know abroad sounds dodgy but he’s a class surgeons registered in the UK and is very well known. Visits the UK a lot for conferences etc. Pioneered a technique with less scarring etc.

I had pain post surgery, some people are fine but I woke up in recovery screaming, I could feel the drains and so they pumped me full of morphine. Went home after three days but was in agony for a while. For a couple of months I lived on puréed baked beans, cornflakes and soup. Some people find after a couple of months their restriction eases and they can eat more - never happened to me but I also now have a mesh around my oesophagus because I had a hiatal hernia (repaired during surgery).

You have to chew more than you would have done in the past. If you don’t chew enough and it gets stuck at the top of your new stomach, you’ll experience the foamies. This is where your body produces “goo” to move food down the tract but because you now have a tube for a stomach, the goo can’t lubricator around the stuck food so it just piles on too and then you feel the need to burp but really you just need to bring it back up to relieve the sensation. Once you get to goo point you will be sick but it’s not the same sick you’re used to, can only describe it as regurgitation. You won’t feel full in the same way, it’s just pressure on your chest and if you’re lucky you’ll hiccup. That’s when you know you cannot take another bite, I think the hiccups help move the food down. Basically those things will happen if you don’t follow the rules. I categorically eat too fast everytime so I don’t think I’ve stretched the stomach because I never fill it as I’m constantly like oh shit. I used to be “sick” after every meal, now it’s not that often. Might have a week where I do it a lot but I’m used to it.

Say goodbye to chicken! There are certain foods you’ll no longer tolerate, chicken breast is one of them. Slow cooked chicken thighs are fine. You know how when you chicken chicken breast it kinda balls up? Any food like that you should avoid. I can’t eat bread but I can eat 3/4 slice of toast. Can’t eat yoghurt too fast, can’t eat a whole magnum ice cream. Can eat a slice of pizza. Can’t eat to much broccoli or cauliflower as it will hurt my stomach. You’re not supposed to drink fizzy drinks but they help me tbh in a weird way. The bubbles help me not want to eat, diet ones that is.

I sometimes have hypoglycaemic moments and have learned to avoid things like cake with sugary syrup ie lemon drizzle because it’s like the syrup hits me all at once and then I have a sugar low and pass out at work (happened 2 weeks into a job). I’ve learned my gut is sensitive to sugar and if I take the piss I’ll be very uncomfortable. As you’re taking in less food, you’ll poo a lot less but you have to make sure you drink enough or it will be like passing boulders. Sorry I’m telling you all the weird stuff!

You have to have b13 jabs and take various vitamins. Because I hardly eat anything I’m on a full spectrum of iron, b12 vitamin d etc

Your hair will fall out as you loose weight; so I had the chop as it was a annoying. Hair will get back to normal after a while, it happens with any weight loss method that is fast.

I pretty much eat what I want just in smaller amounts. I have porridge for breakfast, but I drink my coffee first I can’t have it afterwards. I snack on babybel, olives, deli meat. Lunch I eat leftovers or I have a slice of toast. I can almost eat a full mugshot. Afternoon I’ll have fruit or crackers, can eat two crackers. Might have a biscuit. Dinner I’ll eat whatever the boys are having for dinner. Just in tiny amounts. We had ribs the other night, I managed two and a piece of broccoli. I would say, I don’t enjoy food now but I enjoyed it too much to start with. If DP wants takeaway, he can choose - what’s the point in me choosing. If I cook, I’m not cooking to please me because I can’t eat much.

That sounds really negative all above it’s not really it’s just a different way of living now. I avoid foods that cause issues and I eat small amounts of food. I still eat naughty food, but I’ll feel sick if I eat too much so I can’t binge.

I have a lot of saggy skin, my bum looks like a shar pei so I’m saving up for cosmetic surgery. Arms, legs, bum, boobs, tummy. Omg and under my chin is baggy :(

Oh, didn’t keep it a secret who cares. It will be pretty obvious that you’ve done something to lose weight that quickly. I met my partner when I was 16 stone and was surprised he fancied me as I’m a bag of skin and had such low self esteem from years of being told I was ugly. But he says it’s like I’m wearing a baggy jumper and as weird as that sound he’s right, because I’ll have surgery to fix it one day

To be clear, I wouldn’t change this - I should have had surgery earlier. It won’t fix your head, if you have an emotional relationship with food you will need CBT or something. Surgery fixes your body only.

As an aside, I also divorced my husband because it gave me the confidence eek

Took me 11 months to lose 7 1/2 stone. Maintaining now for 6 months, I did a VLCD & have changed my life.
I had been considering weight loss surgery but this was the better option for me.

Seriouslyconfused3 · 10/06/2020 15:01

@Yorkshiremummyof1 you look amazing! Thank you for your honest insights- I’m trying to mentally prepare as much as possible before hand! Did you get much criticism from people for doing it? I come from a very judgey family Confused

Yorkshiremummyof1 · 10/06/2020 16:05

Nah they are happy I’m thinner. I lost a few friends, one in particular said I was selfish and only cared about how I looked. She was right to a degree, I was very concerned with my changing looks but she also forgot I went through a bit of abuse and divorce, conviniently forgot that..... for all the stuff I said that sounds bad, nothing beats fitting into a dress that I couldn’t have dreamed of wearing before. Also, my king size bed suddenly seems huge! Before it felt like a small double, now we have space for the cats to sleep between us hah

Seriouslyconfused3 · 10/06/2020 21:12

I can’t wait to have it and feel some change (no matter how small). Can you recommend any protein shakes or waters?

MonkeysandParrots · 10/06/2020 21:32

@Yorkshiremummyof1 thank you so much for sharing your pics as well as the good, bad & ugly on the journey you’ve been on. You look brilliant.

I had my consultation today, it went well, surgeon was lovely. Dietician and Psychologist booked for week after next, surgeon says aim for late July/early August for the op.

disorganisedsecretsquirrel · 10/06/2020 22:32

Best thing I ever did.
Will be forever grateful to the NHS.

Before : size 24
5 ft 6
20 stone 4 ills
Sleep apnoea
Severe arthritis
Couldn't walk 50 yards without stinking back ache
Pre diabetic
High blood pressure
Low immune system and at the doctors every 6 weeks for one thing or another...

Last 6 years I've seen doctor 4 times. Only for routine thyroid tests.

Have no diabetic symptoms. No medication for it now.
No arthritis
I walk EVERYDAY a minimum of 10k steps but often for twice that.

I completed a half marathon last year
Arthritis pain has disappeared.

Best of all No sleep apnoea. I sleep all night.

Go on YouTube and look for Professor Rachel Batterham. UCL obesity specialists and she will explain why diets don't work once your BMI gets over 35.

Seriouslyconfused3 · 11/06/2020 07:39

@disorganisedsecretsquirrel how much did you lose and over what time period?

Could anyone share an example of their daily diet now?

OP posts:
Yorkshiremummyof1 · 11/06/2020 10:45

I just use PHD Diet chocolate protein powder. Prefer one with less sugar etc. I have one a day with banana.

disorganisedsecretsquirrel · 11/06/2020 14:30

I had my gastric sleeve in an nhs hospital in December 2012. I lost 7 stone by July. (3 stone in first ten weeks) . I plateaued for a couple of months.. then lost another stone and a half over the next 6 months. I am now 11.3 BMI 26.

Breakfast. Cup of coffee with a Splenda (still can't drink unsweetened tea/coffee.
After half an hour or so I have a weetabix with milk.

Mid morning I have a yoghurt with some fresh berries (raspberries/blue berries) or make smoothie with the same.

Lunch is usually a Tuna Mayo revital or tomato, mozzarella salad with basil and balsamic vinegar. Yum.

Afternoon is hummus and carrots or celery

Supper is usually Thai stir fry /green curry, jasmine rice or baked potato. (But the portions are small - side plate sized).

This is six years in though. I have a normal diet. I don't deprive myself. If I feel like chocolate then I have some - but instead of a family bar of fruit and nut I have a couple of squares of high cocoa plain.

It's about re educating your food choices. Eating until you are sated but not stuffed.

Drink water . Lots of it. .. NEVER EVER GO NEAR FIZZY DRINKS . The gas expands your stomach and makes rooom for more food.

If you want an alcoholic drink then vodka and flat slimline tonic is my go to. But weight a year or two.

Yorkshiremummyof1 · 11/06/2020 14:36

Okay food, I don't know portion sizes but assume with cereal and porridge its half the normal amount, crackers or biscuits its two max and I'm stuffed, crisps its half a packet you get the idea

Breakfast
Porridge/Cereal/Yoghurt and berries/Smoothie or Protein shake with banana and almond butter/PBFIT powder/avocado

Lunch
Mugshot as I'm lazy. Might eat leftovers but we're talking two spoons of rice, two spoons of curry. Small salad. Might eat half a wrap from tesco. Mug of soup, thats in terms of how much I can manage. Depends what it is made of!

Dinner
So I'll eat ANYTHING for dinner, I don't eat particularly healthy
Two fish fingers and maybe 6 chips?
Bolognese, two spoons of pasta, two spoons of sauce
Ribs and potato, three new potatoes and 2 meaty bits from the ribs
Pizza - one slice, probably have a second in half an hour
Half a salmon fillet, some roast veg

Assume I eat what other people do but 1/4 the normal size.

I can tell you I don't eat...fatty food such as takeaway curry, food spends less time in my stomach and enters my gut quicker. Highly fatty food results in....fun. Not long after my surgery I was feeling miserable so ate half a tub of ben and jerrys, half an hour later I was literally moaning on the toilet (sorry). I can eat almost a full magnum but thats my limit, any more and I'll be sorry. I do eat lentils and beans but I don't eat loads because I can't handle loads of fibre, I need to balance it out. Sometimes I end up only eating the meat at the dinner table, plus maybe a potato so I take fibre tablets as well.

I don't eat too much sugar at once, I'll "dump" which is a gastric bypass thing but it kinda happens with a sleeve, its basically blood sugar levels dropping insanely fast, causing me to sweat, feel sick, then need to sleep.

I avoid chewy food, like I've said before chicken breast. I can eat it sliced up though. I can eat 1/4 of a rump steak! Yay

Crisps, pringles make me throw up if I eat more than 5

I can eat chocolate but after too much I feel weird, not nauseas but I feel a bit sickly. I can eat two squares of dairy milk safely, any more than that and I don't feel good. Same with Peanut Butter cups, any more than one and I need half an hour to recover.
The trick is really to eat the most nutritious food. I have to take so many tablets a day because I'm anaemic and well deficient in everything. If I don't I have spates of really bad dizziness. Smoothies are good for getting stuff in

needtoloselots · 12/06/2020 08:50

Can I join in? I'm also considering weight loss surgery. I've had a quote by email, I just don't seem to be able to pluck up courage to pick up the phone and talk to someone.

Seriouslyconfused3 · 12/06/2020 09:16

Hi @needtoloselots I felt like that too! Put it off for ages- I was nervous of judgment if I’m honest but they were all so nice. Where about are you and what surgery are you thinking of?

OP posts:
needtoloselots · 12/06/2020 10:49

I'm in the midlands, wheee are you? I'm considering either the bypass or sleeve. Although the sleeve seems a more likely option. I'm most nervous about having the anaesthetic though.

Yorkshiremummyof1 · 12/06/2020 10:54

Fizzy drinks don’t expand you stomach it’s a myth, too much scar tissue. It’s more to do with insulin and the fact that fizzy drinks move food through stomach faster. I still denim fizzy, in fact almost exclusively fizzy water as for some weird reason still water makes me feel sick (no idea why )

Seriouslyconfused3 · 12/06/2020 11:47

I’m in the north west I’m booked in for Nottingham bmi for a sleeve. I did want the bypass but the dr said that as Im not too extreme weight wise the sleeve would be a safer bet for me- so I was happy with that. I’m nervous too- I keep crying when I think about saying goodbye to my kids Blush

Well done YOU !!

Get off the diet merry go round..

The only people who lose £££ are you..
the diet industry is VERY anti wls for this exact reason. They want you to keep losing and gaining !

titsaleena · 12/06/2020 19:59

I considered this myself but as I know 5 people who have had this done with various problem for each, i was a bit scared.

You sound like me. I've done every diet under the sun, numerous times. With each one I end up bigger than I started. 38 years of this bullshit and I've had enough.

10 days ago I started something I'd never tried before- intermittent fasting following a pattern of 20 hours fasting and 4 hours eating. I've eaten everything I wanted in the 4 hours- carbs, dessert etc. I've lost 6lbs and it's actually been piss easy. It's never felt easier in my life. There's a book called delay don't deny by gin Stephens, PLEASE read it. There are Facebook support groups as well and the stories on there are incredible. Also the obesity code book.

I know you didn't ask for diet advice so I apologise if this annoys you. I just feel like a something has changed in me. I'm a severe emotional binge eater. I was eating all day every day and a lot of it in secret. I have never felt this positive before.

Good luck with whichever path you choose!

I wish you luck

Cuddlecouch · 14/06/2020 08:15

I am also looking to have the sleeve this year. It was supposed to be in March but everything is currently on hold. My question to those who have had the op is can you list the supplements you take and dosage?

Also my biggest worry is losing my teeth. I have been researching and planning this for 3 years and I have spoken to people who have had the op. So many of them lose hair and teeth. The hair grows back but they have crumbling teeth due to not having enough calcium in their diet. Has anyone experienced this?

Finally, if you went private or abroad were your gps on board with ongoing support. I've read some gps don't treat you or offer the annual blood test if you go abroad for surgery or have it privately in the UK. Thank you for starting this thread op it's been very useful.

Seriouslyconfused3 · 14/06/2020 08:39

Thanks @Cuddlecouch I’d never considered the dental effect Shock

OP posts:
BalloonSlayer · 14/06/2020 09:39

Osteocare do a liquid calcium supplement, a couple of spoonfuls a day, that must be easier to absorb than tablets. Surely as long as you are prepared for deficiencies and take supplements you should be ok?

Yorkshiremummyof1 · 14/06/2020 09:50

I’m out for the day but I can tell you what I take and you can go google it to see what the dosage is as an idea.

Happy to share skin photos of you’re worried about that, arms legs tummy blah blah. My arse is the worst, but I could direct you to my Instagram actually which is don’t touch my jiggle

Adcal d3 - calcium carbonate and vit d x 2
Multivitamin x 2 (prescribed forceval but it makes you sick so boots is fine)
610mg of ferrous Fumarate (it’s iron but in a different form and I take more than other people)
Omeorazole for acid but don’t need that everyday anymore
Folic acid x 2
I also take b12 as I hate injections
Take fish oil, bio cultures and D-mannose for urinary tract health. These aren’t prescribed but I take fish oil for hair and skin, bio cultures because I’m conscious of that and d-mannose because well, for reasons Grin

needtoloselots · 14/06/2020 18:27

Lockdown has been really bad for me. I have barely been out, sitting at my computer all day instead of standing and teaching. I've put on even more weight but worse my activity level is so low I'm finding I'm out of breath and my back hurts doing the smallest thing. I'm going to phone up next week.
