What to ask at a METABOLIC Consultant appt?

I have an appt with a 'Metabolic' Consultant soon.

Last summer I was sent to A&E with a BP of 207/111. I was admitted after they thought I had Pericarditis (later not sure - the Cardiac Dept here is really poor)
My Cholesterol was high & the fasting breakdown was high too:
total chol of 7.8, LDL of 5.5, HDL of 0.8, Triglycerides of 3.8
Told I have a 'fatty liver' (no more info) Age is 58 & female. BMI is 38.
(I had Roux en Y surgery in 2018 which worked well to start with but since then I have gained all the weight back, despite eating carefully. I understand that this is the case for a significant minority of people who have had this surgery and I think my metabolism is completely shot now)
Plus, I've been through a very stressful couple of years which may affect things.

For last 12m I've been given BP meds & 80mg Atorvastatin (caused big issues, had to discontinue). Tried various statins, problems with all (I have mobility issues & they've all caused unmanageable muscle pain)
I've had a test for FH but it was negative but my BP & bloodwork remains high.

I want to improve my health. I asked my GP for a referral to weight management last Oct. It has just been done and the waiting list is over a year from now.

The first time I saw this Consultant I was in & out of the room in 6mins.

Can anyone suggest what I should ask him? I'd like to get the best from the appt to try to improve my health?

Do you mean Diabetes & Endocrinology Consultant?

FortnamsTea · 03/08/2026 13:51

@GingernautI do t know, sorry. My hospital only has a 'Metabolic' dept. One (p/t) Consultant. No other options (in Scotland, cannot be referred on unless your Consultant agrees)

Have you heard of insulin resistance? That may be worth reading up on and may trigger some relevant questions about you as an individual. Ben Bikman has some good videos on YouTube about this area - he's a scientist rather than a grifter

Here's one of his videos on fatty liver

There are some clinical trials at the moment using weight loss injections to see if they help fatty liver disease. That may be worth discussing.

Good luck and let us know how it goes.

I only saw a metabolic consultant once and she ordered lots of blood tests. I had 13 vials of blood taken! So be prepared in case that happens.

You are possibly diabetic so you could ask for an HbA1c test to check. Could you buy weight loss injections privately? That would hopefully bring down your LDL and BP.
If not you could ask for the cholesterol lowering injection which can work well on those who cannot tolerate statins.

@30DegreesHighAndRisingthats really helpful - thank you. It's tomorrow, so I'll 'report back' soon x

@MimiGC Yes, for 5 years after the roux en y I had an annual blood test that was similar!

@sunshinehappydaysthat's helpful thanks. I'll ask (I've been told nothing to date) I know that the roux en y us supposed to work in 2 ways: one it dramatically reduces the size if your stomach (but this gradually stretches to become more normal,) and 2 it is supposed to make you sensitive to sugar so you get 'dumping' if you inadvertently eat lots. I didn't get that effect at ALL. (I try not to eat sugar tho)
