Is it worth having insurance in these circumstances?

My lovely cat is 18. This year's pet insurance has just come through and it is £250 for the year. The excess is £99 plus 20% of any vets fees. I can only claim up to £1,000 per condition and conditions are excluded after 12 months. He has already lost an eye, so any sight/eye problems are already excluded.

If he has to be put to sleep or has an accident/goes missing I get a % of what I paid for him. I only paid £60.

I can't get it cheaper elsewhere.

I can't imagine wanting to put him through any invasive treatments or surgeries in future, as he is so old and it would not be in his best interests. I realise that there are things that could happen e.g. his kidneys could start to fail, and while it feels irresponsible not having insurance, I can't help thinking that there is very little benefit to having it in these circumstances.
