Anyone have a cat with HCM, hyper/hypothyroidism and Kidney disease?

Cat is in the vets - she was admitted Friday as an emergency after vomiting blood.
She has well managed HCM but more recently a longstanding pattern of vomiting got worse and she's been a frequent visitor to the vet for anti vomiting injections and a special diet in case it was a food intolerance.
Anyway - in the process she has been diagnosed with hyperthyroidism and started on meds and her vomiting shot up in frequency, she became very listless and looked 'done-in'. Weds/Thurs and then Fri she was just vomiting alot and so I rang the vet (again) and made an apointment for the evening. Then she vomited blood and vet said bring her in immediately.
Some examinations later they said she needed to be admitted becasue she is now showing hypothryoidism. So she's been in the vet's all weekend having anti vomit meds, scans and other blood tests which show results that look like kidney disease which I understand can be masked by hyperthyroidism until treatment starts and then the kidneys don't get the blood supply they've had before.

Anyway - my own vet said whatever the clinical picture it isn't going to be positive really.
So I wanted to ask any cat owners if you've got any experience in the same clinical picture for your cat. my cat is 13 and a housecat. thank you for reading.

I had an 8 year old Persian with horrendous HCM. She developed kidney failure towards the end. But was only diagnosed the day before she was PTS. With hindsight the signs had been there for a couple of months. The cardiologist doubling her diuretics tipped he over the edge. The vet said it was treatable. But I knew it was not. I looked at her and knew it was time. So I waited for the blood results to come back to confirm it. And had her PTS. I have no doubts it was the right thing to do.

If your car was mine. I'd let her go. Very sorry.

Surely the vet would have told me by now if they thought there was no hope @Toddlerteaplease ? It's her quality of life I'm most worried about. I'm sure they'll have a medical management plan but I think I need more information regarding the conditions and how they're likely to impact her quality of life as well as expectancy.

I think by saying that the picture isn't good. They are trying to tell you gently. My cat was never unwell with her heart at all. (Heavily medicated) But she was unwell with the kidney failure. I just didn't realise until the very end. But as I said, the signs were there. When more than one organ starts failing it's not a great outlook. You really do need to consider her quality of life. She must be feeling pretty unwell.

Just because it can be treated it doesn't mean it should. Sometimes the kindest thing is to let them go.

MadamClench · 01/05/2023 08:29

@Toddlerteaplease I understand but I can't accept that's what might be happening.
Vet just called to say she seems quite happy and is eating and drinking but her renal values haven't changed so they're keeping her in another day until transfer back to my own vet tomorrow. they'll do another blood test tonight because although her renal values were worse when admitted they are still not as they should be and aren't moving.
What I want is for the Vet to be straight with me and to not say well you could do this, or this, and leave the decision totally on my shoulders because what if I make a mistake and have her euthanised when it's not necessary?? I'm all she's got and she's all I've got.

Better a day too soon than a week to late. I know it's really hard. But both the heart and kidney's are one way problems.

@MadamClench I truly feel for you - my beautiful boy was PTS last week and I understand the agony you are going through.

However, no vet will tell you what to do - they will guide you to make the right decision and if you ask the "right" questions you will know. Ask them and listen to what they say carefully.

I know it is a horrendous time and you think it is unbearable but this is what we owe them - making tough decisions (when the time is right) and showing them the ultimate kindness.

I agree that a day too soon is far better than a week to late - but it's easier to say than do.

Stay strong and you will know what to do - she loves you and she knows you care.

I just thought I'd write this.
She had another blood test and we went back today for the results.
She has Thrombocytopenia and the vet very kindly but firmly told me that really there is nothing they feel they can do for her now especially given her age.
I've brought her home with some pain relief and prednisolone and she will be euthanased on Saturday.

So sorry to hear this. But it's the right thing.

So sorry, it's a very loving gift to release her so gently.
