Elderly Maine Coon is in pain - what next?

Two days ago our 14-y-o Maine Coon (who is an indoor cat) didn’t wake us up for her breakfast, and we found her hiding under the sofa. I took her to the vet, who said she’s lost 20% of her bodyweight, and took bloods, but said it’s probably musculoskeletal, perhaps a trauma from a fall, or arthritis, and dosed her up with Metacam, which perked her up no end and she was eating and interacting that evening.

The blood results, which came back yesterday, showed hyperthyroidism (which might explain the weight loss, and for which we now have medication) and early signs of kidney disease (for which we need to change her diet), but no markers for infection or cancer. Vet says to keep her on Metacam for a few days, which will help her while she recovers if it’s a trauma pain, and then put it on the shelf and see how she does without it.

When I tried to brush her yesterday (extremely gently) at one point she suddenly hissed, ran, jumped onto the cat tree, pooed and was clearly in a lot of pain.

As it was almost 24h since her first dose of Metacam, I gave her the next dose, and she perked up very quickly. That was at 5pm yesterday, and she ate her tea and seemed fine all evening - interacting, vocal, demanding.

This morning she was hiding again, had just done a poo on the carpet as I was looking for her, and is still hiding, and hasn’t touched the breakfast that I’ve placed nearby in her hiding corner. So I assume that the pain-relieving effects of the Metacam barely lasted 12 of the 24 hours.

I’ve called the vet and am waiting for a call back, but what do you think are likely to be the options at this stage? Up to Tuesday she seemed absolutely fine, so this has come out of the blue, but obviously she’s quite an old girl now.

Yesterday the vet seemed happy enough to give her pain meds for a few days and then see if she manages without the meds (which would suggest acute pain, eg from a trauma, though she’s a sedate indoor cat in a calm household) or if she doesn’t, it’ll point to arthritis. However, if the Metacam doesn’t ‘last’ 24 hours, she’s going to suffer for part of every day, and we absolutely don’t want that for her.

Is there anything else we can sensibly ask the vet to do, such as an X-ray or scan? We would like to know what’s causing the pain, rather than guessing! She’s fully insured with Petplan and we’re willing to give her every chance, but we would of course end her suffering, if it was the right thing to do.

I've had a call back from the vet. I'm to give her her next dose of Metacam at midday (5 days early) and call him with a progress update at 2pm.

He said there's no point in scanning her, as they would be unlikely to act on the results (she's too old for spinal surgery, for example). So the only option is pain management or, if we can't get that under control, euthanasia :(

There are other pain meds. I went through similar with my last cat, he ended up on Onsior for his arthritis, it's supposed to be gentler on kidneys.

I'd ask for a second opinion, your vet doesn't sound very motivated. A scan is needed I reckon for a potential diagnosis. Then, suitable pain relief can maintain her quality of life for as long as possible. You're insured. This is what it is for.

She's had an injection of something ultra strong, and I'm to call back with a progress report at 5pm. If she has started to move/eat/engage by 5pm, I'm to pop back for more of the strong stuff but tablet form. If not - decision time.

The very good news is that she is currently chomping her way greedily through her delayed breakfast...

GrrrlPwr we actually did! It theoretically hides the hairs a little... picked to blend with her coat and her brother's (pictured). We sadly lost him last year.

I hope your beautiful kitty is OK. My girl is also 15 and has early kidney disease. She is on special food which is costing me a FORTUNE and its slightly disturbing to see how much she drinkers every day but she's very much herself at the moment bar crap grooming. I get her food from www.bitiba.co.uk/shop/veterinary/cat_vet_food_supplements/renal - it was recommended by a vet nurse as where she gets hers as it's cheaper than the vet and you can't get it in places like Pets At Home.

I would want the vet to rule out causes that can be treated, for example have they looked at her teeth?

My cat had a similar thing, though he's quite young, he lost masses of weight and we couldn't get him to eat. One thing that helped was switching him from a bowl to a plate. I read that they can get stressed out by their whiskers rubbing on the bowl.

We never got to the bottom of what was wrong with my cat, but he's fine now.

Thanks for all the responses. It's good news - our cat has rallied!

We had her on a cocktail of drugs over the weekend (Metacam, Gabapentin and an opioid) to ensure she was in no pain, and she was high as a kite, bless her, but was eating, using the litter tray and appreciating attention, so it seemed all was not lost.

On Monday, we took her off everything apart from the Metacam, and she has gradually perked up, and is now behaving pretty much as normal - vocal, hanging out with us, eating reasonably well etc. I'm waiting for the vet to confirm that we should try her without the Metacam!

I honestly thought there was no hope, but I am very pleased to have been proved wrong. We get to enjoy some more time with her!

I thought I would post an update. It's sad news I'm afraid.

On the Friday (6th) our lovely girl was once again hiding behind the sofa. When we managed to fish her out, she was clearly once again in great pain - she didn't want to move at all, and was shuddering slightly with each breath. I gave her a Gabapentin and the emergency syringe of opioid to alleviate the pain, and took her on a one-way trip to the vet :(

We are all very sad, but we knew it was the right decision - we couldn't bear to keep seeing our gentle girl suffer.

I volunteer with cats, and one thing I have learned over the years is that it is infinitely better to let them go too soon than let them linger. Cats are very stoic and judging when the time is right is so, so difficult. From your description it is clear that you let her go when it was the right time.
