Old cat

Does anyone have experience with old cat please? Our old boy is almost 21. He's on medication for kidneys and drinks loads and loads of water. He went to the vet on Wednesday as he's been constipated. Vet gave him lactulose to take twice daily. Still no movement just one or two tiny hard pellets per day. He ate all his breakfast and is still after more food constantly. He must be really backed up inside with not going to the toilet for so long. He can't seem to sit down properly and often stands starting at his water bowl, into space or at the wall. He can't settle seems to have trouble staying still and even getting up and down from his bed. He is forever backwards and forwards to his litter tray for big wees. But he still cleans himself and eats like a horse.

Does anyone know how long lactulose takes to work and what happens if it doesn't?

Yes, you got him to 21 which is wonderful. Proud. He must be very loved.
No idea about lactulose in cats, doesn’t it need to be taken with lots of water to work well (in humans)? And he has been drinking lots so that is met.
Hopefully he will make it a while longer.

My old cat suffered with chronic constipation and mega colon. We unfortunately had to make the decison to end her suffering due to this. She was so bad she needed regular enemas under sedation. For us, nothing worked sadly. Miralax helped a little but lactulose, liquid parafin etc did nothing.
If this is your boys first time with constipation then he may need a micro enema. Not pleasant for the cat I'm sure, but doesn't require anesthetic and usually fast acting.
You mention he cant sit down properly. Our old girl had arthritis and spondylitis which meant she couldnt crouch down enough to poo.
I would get him to the vets tomorrow 1st thing as 5 days is a really long time to not go properly.
