Ongoing ear problems - need to vent

My poor dog (7yo GSD) has had problems with her ears for years, and I'm just so sad for her. Does anyone have any words of wisdom / anything else I can try? Honestly, what would you do?
So she was first diagnosed with allergies when she was probably a year old. She has pretty much been on apoquel ever since, which fortunately we can afford. We did try to wean her off at one time during covid, as she caught kennel cough which nearly killed her (as the apoquel acts as an immunosuppressive) but we weren't successful. After several ear infections in a row we relented and put her back on the medication.
The last couple of weeks have been typical of the past few months, but the vet doesn't seem bothered / doesn't think that there is anything else to do. She went in for her booster vaccinations and health check last week and I said her ears were a problem like they always are, and I got the knowing nod. He had a quick look but didn't find infection or anything so let her carry on as 'normal'. Her normal is to generally / daily be scratchy around the ears. Every few days she will have an onset of symptoms where it looks like an infection might start (e.g. scratching lots / head shaking etc) and we have to put steroids in her ears which we always have a store of. Her ears (the inside skin area) seem to be a darker colour than they used to be and she is losing hair on the outside of her ears now. I clean her ears weekly (can't do them more often as it is a big irritant to her and sets her off scratching, which is when we seem to get infections) but the wax is disgusting. I have flagged many times with the vet is it possibly something other than allergies, but always told that it is. We did do tests which came back with several allergens so it does seem to be the cause - some allergens can't be avoided like certain trees and grass pollen, but we got hypoallergenic food to avoid the food allergens. I am at the vets, on average, once every 3-4 months with ear infections. But like I said, she has constant symptoms.
The only other option we have been offered was a couple of years ago and to me doesn't feel a valid option, and the vet said it would only be for extreme circumstances, that they could (as I understand it) remove part of the ear canal. It would stop her symptoms but would obviously make her deaf! If it was always one ear I believe we would have done this by now, but her problems affect both ears.
I have tried elimination diets, assortment of ointments and supplementary tablets etc and nothing has helped. I'm all out of ideas and I just feel helpless.
I can't put my dog to sleep because she has ear infections / allergies......but I'm finding it hard to believe I'm doing best by her when she is so often in discomfort. I'm not neglectful, I've done everything I can, thrown money at the situation, been down the vets so so so many times, always complete the courses of meds given etc......but eventually the infection always comes back.
I'm just looking for a place to vent. This isn't the life I want for my dog 😢

You can get custom immunotherapy injections made for dogs with allergies- I don’t think it’s cheap but might help with the itching

for any persistent problem a pet has, you always seek a second opinion. Just like you would for yourself if a doctor failed to provide adequate care or treatment:

Thank you for your input. We have changed practices twice in the past 6 years, and certainly the one we go to now we have seen at least three different vets.

I would ask for a referral to a specialist. My dog had allergies, itchy flaky skin and ear infections and we saw a dermatologist. He had to be on a permanent low dose of steroids to keep the ear infections at bay. Everytime we tried to reduce the dose, his ears flared up again. He also had monthly immunotherapy injections.

Have they checked / treated for ear mites?

What allergens have you removed from diet / environment?

Have you already tried human antihistamines? Dogs can have Piriton (chlorphenamine) as I understand it.

Just looking at FecLab now for Giardia test kits for my pup, and see they offer skin health tests (Canine Yeast Dermatitis) - and after reading your post this morning- immediately thought of you… unsure if it’s worth a look/read?

Sorry for your poorly pup otherwise , and I’d be looking to change vet’s too x

She might be one of those dogs who really needs a biologically appropriate diet of raw meat (not processed kibbles)

my dog had horrid yeasty ears, looked like black treacle oozing out so bad she needed a surgical flush at only 8 months old 😢

switched her to raw food and problem hasn't come back.

once you start learning about processed food and the effect it has on both our species so much stuff starts to make sense.

I would give raw feeding a chance if I were you OP.

My GSD is the same. We got her as a 5 year old from a rescue who'd been treating her for terrible ear infections. She's on Apoquel, and also Cytopoint right now. Her ears flare up all the time and the only thing that works is regular ear cleaning. We use Chlorexyderm ear cleaner with cotton wool, and got these little finger gloves which mean I can get right in with my index fingers. She gets a squirt of Cortavance steroid spray if they're starting to become red and sore. If it's a yeasty infection with brown or green liquid, then it's Aurimic drops instead.
One time she picked up Pseudomonas from a groomer, and it took a few months and ear swabs to get it under control. The regular cleaning does work though, and she doesn't mind it any more at all.

I've seen good reviews in stopping ear infections from a company called Pick Peanut, but haven't tried them yet. They provide supplements to limit yeast growth.

Our vets are fantastic, we changed from one who I didn't trust, but some dogs are just very susceptible to these infections. Our GSD even got them checked under sedation to see if they could do a more invasive treatment, but it's impossible to see 100% if her ear drums are intact.

Poor bug, that must be so hard to see.

I'm sure you've tried everything already, but just in case - my dog used to have constant ear infections until I removed all dairy from his diet. Since then they've been absolutely fine, not a single infection.
