Obviously there is a fault but this can happen really easily , we had a lead that we had been using for a number of years and it ‘unclipped’ one day a couple of years ago , my dog is well trained so I just told him to stop and he did . I now walk him with a double end lead with both ends attached to the harness just in case . It is a very sad case .

This is one of the reasons both my dogs wear half check/martingale all in ones.
There’s no risk of a clip pinging open and in theory they can’t back out as the chain tightens, the only way they could escape would be if the actual collar broke in half.

Awful, awful story though I have to say, while I’ve never seen one of these leads in person, from the photos I’ve seen in articles about the story, the clip does look pretty flimsy.
Maybe it’s just my eyes but the clip in the photo appears to be plastic?

Also check all your kit regularly and if it ever fails replace it immediately. The saddest part of this story is that the lead had already come undone once. At that point it should’ve been thrown away or taken back to the shop.
The lead shouldn’t have come undone-but it already had once-so should never have been used again.

The clip isn't plastic, it's metal and the reason it came undone is because it is of rectangular section but being attached to a collar or harness ring which is of round section metal. The same issue occurs if using a round section clip lead on a rectangular section collar or harness attachment.

Use type with type and everything is fine; mix them and there can be an issue.

If the dog turns quickly, jumps about a little or even if you are just unlucky, the lead can unclip itself. The first time it happened to me I assumed it hadn't been put on properly but it happened again shortly afterwards when I knew it definitely had.

I reported it to the manufacturers as I came across others who have had the same issue and this really ought to be more widely known. They were somewhat dismissive as they claimed there hadn't been any other reports but perhaps they will take notice now.

Photo is not mine nor my dog but the lead clip/collar ring mismatch is what you need to avoid.
