Any idea what this is?

Our Tibetan terrier had his anal glands removed two years ago as the vet thought they were causing his issues (lots of scooting).
Vet who operated then said dog had dermatitis on the base of his tail and that was possibly reason for scooting.
Last trip to vets, dermatitis was evident and we have steroid cream.
Scooting is back with a vengeance. I can’t see any dermatitis but this is his bottom. it doesn’t look right does it?
Any ideas please?

I am not a vet so have no idea but do dogs get piles? I guess it is back to the vet to get it checked out

No idea -sorry- but you dog is obviously suffering discomfort so take it to the vet.

I once met a dog with piles. He used to scoot, but also used to spend a long time doing a poo, poor sod. He'd be in the crouch position for ages trying to squeeze one out slowly because it was painful. (Well, thats what the owner told me, anyway) Although op, is that scar tissue from anal gland removal?

Dear dog’s pooing is completely normal so does that rule out piles?

Seemingly digs can’t get piles but can suffer from things that can be mistaken for piles.

Vet has prescribed apoquel
There are saliva sores on his anus where he is licking so much.

What food is he eating? If he's having trouble going it can cause discomfort. How are his stools?

I know someone with a dog who has Anal Furunculosis - might be worth considering?
