Yes I'm presuming Joe is sick, probably dying and that's why he came back.

I feel sorry for Ruby, I know she was a cow in the beginning but she's really really good, I remember many months ago she was talking to Chas and made some reference to her childhood, can't remember what but she was very agitated and was digging her nails into the table while she was talking, she was very anxious at the time.
