Duke of Edinburgh Evidence for Skills, Physical, Volunteering

There's another thread on here which is just about the expedition, but I wondered if anyone could shed light on what the children have to produce as evidence for their skills, physical and volunteering sections? My DS has completed enough sessions for all of these but doesn't seem to have done anything about uploading anything. The school is (quite rightly) communicating with the children rather than the parents, but my son does sometimes require nagging support to help him to complete seemingly simple tasks. Do they literally just have to get the booklet pages signed by their activity leader for each section and then hand these in? Do they have to provide other evidence e.g. photos, certificates etc.? Thanks for any help from those who know more than me!

Unless it changed recently then all he needs is a brief description of what he did and when plus the activity leader's signature.

DD is in the school show (that counted as volunteering). She could have done the dancing (her part in show) instead for physical. Drama teacher could verify her part (wrote a very nice comment).

For the physical, she put her regular runs on Strava & (I think) produced the printouts & Strava summary as evidence which PE teacher signed off.

ps: DD did Brownies badges. Getting stuff signed off for DoE seemed a lot easier to me!

Just a description and a signature from what I could gather from DD.
Think her drama leader wrote a comment or two as well.

My dc printed off DoE forms which were sent to us from school (but ar probably on the website).

All he does is lists the date, brief description of what he has done, and then someone signs it off. The description is just one line. For his 'Skills', he is doing cooking, so this week will write 'Rock cakes, using the rubbing-in method'.

I think my DD has a A5 booklet which she's asked her activity leaders to fill in and sign. I haven't seen any photos of her doing the carious activities.
