Skin crisis - advice please!

Hello! Hoping for some advice to get me out of a massive skincare crisis! I'll try and keep it short...I'm 48, skin has never been great, usual signs of aging and I've suffered outbreaks all my life. I had been using Murad retinol range with good results but skin started to break out more than usual and was showing signs of barrier damage round nose so decided to take a break from retinol and settled on Skin&Me to try and clear up all issues at once. Prescription is Azelaic Acid 10%, Clindamycin 1%, Niacinamide 4% so tretinoin at the moment (due to be introduced next month). All was going ok for the first week and it definitely cleared up the outbreak I was experiencing and skin started to feel calmer but I then started using a new Roc multicorrexion night cream (no actives) and I'm not sure if it was the Skin&Me or the Roc or a combination but my skin has reacted really badly. I woke up with a puffy, rashy face which now, 3 days in is becoming scaly and red. I've stopped using the skin@me and the Roc and have paired everything right back and just using my trusted Murad cleanser, Eborian Centella Crème and a Murad cellular hydration mask at night but I'm at a loss what to do next. I'm definitely not touching the Roc cream again but do I ease back to the Skin&Me and persevere with that or do I stop it all together and go for something else? Whatever I didn't like skin&me cleanser and moisturiser so I'll need to add those in. I've used Murad and Medik8 in the past and like both brands but have never been completely without the breakout issue so still feel like I'm looking for my holy grail in skin care regimes. I'm spending hours researching other options and am drawn towards Dr Sam Bunting and REN both of which look good for blemish prone and sensitive skin but equally tempted to return to tried and tested stuff. Any advice would be so gratefully received. I am so fed up with looking a spotty, wrinkly, scaly, blotchy mess!!!

I'm not an expert but if it were me I'd go back to the absolute basics, cream cleanser, remove with cotton wool and then a simple nourishing moisturiser. I'm currently seeing fantastic results with the Cerave moisturising cream. It's really healed my skins barrier and my skin is looking plump, even toned and pretty damn nice.

I think the holy grail of skincare is tretinoin if your skin can tolerate it. I would cut out all actives and switch to tretinoin only. You don't need expensive skincare brands to add hydration with tretinoin-Korean skincare is really good and affordable.
My skin is so much better and I only use tretinoin each night with lots of Korean hydration products plus SPF each morning. I don't need foundation any more.
I would recommend this Facebook group too if you are thinking about using tret.

Personally, I'd take it back to basics for a few weeks for everything to calm down. My pared down skincare routine would be:

Bioderma micellar water
Cerave hydrating cleanser
La Roche Posay Toleriane sensitive moisturiser or for something more lightweight Byoma gel moisturiser
Hero spf

Once skin is a bit more settled, it might be worth adding in some actives e.g.
Cos de baha salicylic bha liquid (I sometimes use this rather than micellar water for a first cleanse)
Beauty of Joseon retinal eye cream (I use this all over my face at night only)
