What to do about really stubborn milia

One of my teen sons has a large milia on his cheek. It's pretty obvious and bothers him. The rest of his skin is nice and he gets a few spots but thats it.

I took him for a facial but the therapist said she could not remove it as it was 'rooted'. It did bleed due to the attemt to get it out.

He tried to remove it by nicking the skin (I thought it would pop out) but it wouldn't budge.

What can he do next? Is there anything topical? Are there therapists who can remove this or does he need a dermtologist. I'm in London so any clinic recommendations would be great.

I go here for facials and they are great. You can book in just for milia removal which will save some money.

Is it definitely a milia not a cyst? If in doubt I'd suggest a dematologist.

Hot flannel compress for at least ten minutes over a period of days,then nick the top off with a sterilised needle and compress again. Hot compresses are easential ime.

It is definitely a milia and not a cyst but it is like 3 smaller milia joined together to form a larger one. It is really stubborn and will not budge - its not like those spot popping youtube clips where they just pop out. It needs someone who's not afraid to dig it out!

Is there anything topical that would loosen or soften it prior to removal?

I've gotten rid of a few stubborn ones by using a a regular gentle salicylic peel followed by a clay mask. Did it twice a week for a few months and they gradually sank away to nothing. And no new ones have appeared. So now I do it once a week to keep on top of things. Also stopped using skincare containing silicones.
