20yr old with complex needs - support plan.

My son has a community based care package. We have a very good social worker and a detailed, appropriately funded plan (so far so good). The agency we use works reasonably well day to day (there is little choice/space locally so we have to stick with them). However we recently saw the support plan produced by the agency - they wanted it signing but we haven't because it is sketchy, inaccurate, full of spelling mistakes looks as if it was written in 10mins. We are going to work with them and the social worker to get it right. Anyone know what these things should look like - the amount of detail etc.

There’s no one set way support plans should look and some providers include more information than others, but it should be accurate and include enough information to provide appropriate care.
