Care Assessment

Hi there, we have a "care assessment" for ds1 (22) tomorrow. I use inverted commas because its the 2nd time we've met with this social worker and I thought she was doing a care assessment the first time but looking into it the meeting didn't meet any of the criteria for a needs/care assessment.

Can anyone sign post me to any resources that will show me what a care assessment should look like. DS is desperate to leave home but we don't feel he can live independently - may need supported accommodation.

Thanks

Hear me out on this, Age UK’s information is good. Obviously young people have somewhat differing needs to elderly people, but their information about care assessments is still useful.
