Already stuck on the first question. List conditions and approximate start date. If it’s autism, adhd, learning disablilities, should the start date be since birth or when there was an official diagnosis? (Don’t even know if I still have letters for that so couldn’t say exactly when).

I don't know the answer. But what I put for ds and me was, started noticing signs on x date, diagnosis on y date. It's never been questioned. Since it's about what's happening now, I don't know how much they take into account the history dates.

I put “from birth, formally diagnosed on X”. If you don’t know the exact date put the rough month/year or even just the year if you can’t remember. It is about ensuring the conditions meet the length of time criteria. If you want them the GP should have copies of diagnosis letters.
