RAMP form?

SENCO have been in touch with me today saying that they've done a RAMP form with DD and sent it to me for checking or in case I want to add anything.

However I'm feeling a bit out of my depth as I've never even heard of one before let alone know what a good one looks like.

Anyone have any experience?

RAMP stands for Risk Assessment [and] Management Plan. Everywhere sets out their’s differently, but generally the common themes are you should make sure all the potential causes of concern are included, triggers/what increases the risks, how to lower the risks, and how people should respond. Some include what professionals are involved, when it will be reviewed, and who will liaise with parents.
